- The Louis Stokes Cleveland VAMC, Cleveland, Ohio has a requirement for Recycling Services for the Louis Stokes Veterans Affairs Medical Center (LSVAMC) Brecksville and Wade Park locations. The contract period is for one year from the date of award plus four option years. This requirement is subject to the availability of funds. Request for hard copies of the solicitation must be in writing and sent to the Attention of: Joylyn Winter, joylyn.winter@med.va.gov 440-526-3030 X7424 or fax 440-838-6052. The Recycling Services shall include: CLIN 0001 Pick up and Recycle White Ledger Paper 5 Tons CLIN 0002 Pick up and Recycle Sorted Office Paper (Mixed) 8.5 Tons CLIN 0003 Pick up Hard Mixed Paper. To include all paper, colored, etc. 50 Tons CLIN 0004 Pick up and Recycle Prepared Bales of Corrugated Paper Twice Monthly and/or as required at Wade Park Location. 4 to 6 Bales per week CLIN 0005 Furnish Lockable Wheeled Recycle Containers at Brecksville and Wade Park Divisions on an Even Exchange Basis. Cost of rental and hauling of containers to be deducted from this price. 18 to 20 ea. per location CLIN 0006 Pick up and Recycle aluminum cans 520/yr CLIN 0007 Pick up and Recycle Dumpsters full of cardboard at Brecksville (un-baled). 2 dumpsters per week Pursuant to FAR Clause 52.212-2, Evaluation – Commercial Items, the Government will evaluate the proposals based on the following: 1) Technical Capability: Offerors shall provide evidence of general capabilities, key personnel (Project manager and on-site representative, if different individuals), and key subcontractors. Prior to Award Offeror must be registered with Contractor Central Registration (CCR) www.ccr.gov. See Section II Contract Clauses - 52-212-4(t) for further information. a. General Capability: Explain your understanding of the project work, work site, and how you will accomplish the work. b. Business History and Qualifications: The Offeror must provide a complete history of its recycling experience, including but not limited to the following: i. Nature of the company (corporation, partnership, etc.). Ownership, officers, and key personnel ii. Years of experience in the recycling business. iii. Experience similar to the services requested in this proposal. c. Equipment necessary to provide the services required. d. Timeliness, ability to respond in the time required (24 hours after call for pick up). 2) Past Performance: Offerors shall provide evidence of experience providing services of similar size and scope, and shall provide points of contact. The evidence shall include information and examples showing the ability to complete past similar contracts correctly and in a timely manner, and the ability and knowledge of personnel to manage a contract of this type. The Offeror shall provide three (3) references with Points of Contact for Projects of similar size and scope. 3) Price: Evaluation of the Cost proposals considers only the total price for the Base Bid. 4) Period of Performance: Commencing February 1, 2005. Technical and past performance, when combined, are greater than price. Offeror responses are to be E-mailed to the Contracting Officer 90C, Louis Stokes VA Medical Center, Attn: Joylyn Winter (joylyn.winter@med.va.gov), followed by a mailed hard copy. Offers are due by 4:30 p.m. E.S.T. on January 18, 2005. Faxed offers are acceptable as long as the Best Value selection criteria is met and received by the RFP due date (Fax 440-838-6052) and are followed up with a mailed hard copy. Late offers will not be considered. Any questions must be submitted by E-mail and referring to RFP 541-009-05 in the subject line.
