- This is a request for Standard Form (SF) 330 - this is NOT a request for proposals. The VA Medical Center, Tomah, Wisconsin is seeking the professional A/E services to provide working drawings and construction documents to convert three existing deep wells for alternative energy utilization, install water lines to connect existing wells #3 and #4 to existing piping located in Building 400, change/modify existing pumps and install additional piping, wiring and controls, as necessary, within the three existing well houses. The NAICS code is 541330. Estimated value is less than $25,000. This is 100% small business set-aside. By definition in Federal Acquisition Regulation (FAR) Subpart 19.1002, Emerging Small Business is defined as a small business concern whose size is no greater than 50% of the numerical size standard applicable to the North American Industry Classification System (NAICS) code. Although this procurement action is restricted, small businesses, veteran-owned small businesses, service-disabled veteran-owned businesses, HUBZone small businesses, small disadvantaged businesses, and woman owned small businesses are strongly encouraged to submit (four copies of) SF 330 as requested. A/E services shall include a study of the existing deep wells to verify capacity, conditions, etc. The design shall conform to VA design criteria and shall be based on the necessary site vists, surveys, verification of as-built drawings, schematics (if required), accurate cost time estimates, construction period services and submittal reviews at 25%, 75% and 99%. Design shall be complete no later than April 1, 2005. Updated as-builts will be provided on Mylar and AutoCad 2004 drawing format on high-density diskettes. Standard design criteria established for the Veterans Affairs Facilities to be followed; are available at www.va.gov/facmgt/ae. Firms within 200-mile radius of Tomah, Wisconsin are invited to submit four copies of SF 330 to the above named Contracting Officer no later than January 10, 2005. Firms will be considered as to location, technical expertise and other requirements. Facsimile copies are not acceptable; however if available electronically they may be submitted to Lydia.Falk@med.va.gov. If submitted electronically, the Firm is responsible for ensuring delivery. Evaluation factors for consideration of qualifications listed in descending order of importance are: (1) past performance on similar projects, maintaining project schedules and project budgets; (2) Overall engineering design experience and capability with architectural, mechanical, and electrical renovation projects; (3) Qualifications of staff to be assigned, with special emphasis on field personnel to be utilized; (4) knowledge in design and construction requirements; (5) Cost control effectiveness; (6) Location of office supporting project; and (7) Ability to produce all plan documents on a CAD system fully compatible with Autocad 2004. A/E firms which meet the requirements listed in this announcement are invited to submit four copies of the Standard Form 330. Each offeror shall identify past or current contract/agreements for efforts similar to the requirements stated. The past performance information can include, but is not restricted to, performance information regarding predecessor companies, key personnel who have relevant experience, or subcontractors that will be performing major or critical aspects of the requirement. In the case of an offeror without a record of relevant past performance or for who information on past performance is not available, the offeror may not be evaluated favorably or unfavorably on past performance. The area of consideration will be limited to the firms within a mileage radius of 200. North American Industry Classification System (NAICS) codes is 541330.
