Description
The following is the Statement of Work (SSNSOW112304) that applies to the Sources Sought Notice Reference Number SSN1123 that was posted on 11/23/04. The Hatfield Clinical Research Center houses the inpatient units as well as research laboratories. It is scheduled to open completely in January 2005 and has limited use at present. Specialized housekeeping services are required for the Clinical Center Complex. The contractor will provide specialized cleaning in the below described areas. The contractor shall have passed two consecutive surveys of the Joint Committee for Accreditation for Healthcare Organizations (JCAHO) with no deficiencies in the housekeeping department at a present client hospital of more than 300 beds. This is necessary due to the various types of tertiary acute medical care provided and the number of rare diseases treated at the Clinical Center. The Clinical Center has an exemplary record of JCAHO surveys and cannot jeopardize its long standing record. Contractors shall comply with all applicable federal and state government regulations regarding control of infectious diseases in health care facilities. Contractors should consult the specific regulations for further details, exemptions, documentation, and record keeping requirements. The contractor will be responsible for the cleaning of approximately 1,096,634 square feet of space that includes patient care areas, laboratories, exam rooms, public spaces and offices. See attachment #3 for the complete list of areas and the approximate square footage. The areas include 483,683 square feet of patient care spaces, 87,394 square feet of acute care patient areas, 301,220 square feet of office and public space and 224,337 square feet of laboratories. A detailed listing of the spaces is attached. The areas to be cleaned are the complete CRC except for the laboratory wings, mechanical and animal areas. The areas to be cleaned in the Clinical Center are the third through thirteenth floors of the Ambulatory Care and Research Facility (ACRF) wing which include a clinic and a laboratory suite on each floor, the complete first floor of the Clinical Center which includes the main lobby, nuclear medicine, radiology, special procedures, blood bank, Phlebotomy, travel and voucher offices, safety offices, echo lab, MRI, Clinical Research Volunteers, film library, ultrasound, CT scan, and all other rooms and common space. The contractor shall clean all hallways that are in these areas. The cyclotron on the B2 and B3 levels shall be cleaned by the contractor. The contractor shall clean the Clinical Pathology labs and offices and all other common areas on the second floor of the ACRF building. The contractor shall clean the Material Management offices and supply area as well as the sterile supply area on level B1 of the hospital building. The contractor shall clean all stairwells and elevators in the ACRF and the CRC. The contractor shall have employees on duty 24 hours per day, 365 days per year to service all areas designated. Cleaning will include but not be limited to: Patient Rooms: 1. Remove trash & clean receptacle Daily 2. Perform High Dusting Daily 3. Wipe horizontal surfaces including bed, stand and tray stand Daily 4. Spot wipe walls Daily 5. Dry mop floor Daily 6. Wet mop floor Daily 7. Clean bath including bath, bowl, shower, and restock supplies Daily Clinic Examination Rooms and Procedure Rooms*: 1. Sanitation of exam table and furniture Daily 2. Wipe all vertical and horizontal surfaces Daily 3. Remove trash and clean waste receptacle Daily 4. Spot wiping of walls Daily 5. Cleaning of vents Weekly 6. Dry mop floors Daily 7. Wet mop floors Daily 8. Wall washing Monthly 9. Floor Stripping Semi-Yearly 10. Perform high dusting Daily 11. Floor burnishing Twice weekly * Procedure rooms must be cleaned after each procedure. The cleaning system used by the contractor should be the same as the cleaning procedure for a surgical room. Laboratories: 1. Remove trash and clean waste receptacle Daily 2. Spot wipe vertical surfaces Daily 3. Clean vents Weekly 4. Wipe cabinet fronts Daily 5. Clean floor Daily 6. Floor stripping & refinishing Semi-Yearly 7. Floor burnishing As Required Public Rest Rooms: 1. Clean & sanitize all fixtures Daily 2. Replenish all bathroom supplies Daily 3. Clean vents Weekly 4. Spot clean walls Daily 5. Clean floor Daily 6. Wall Washing Daily All public bathrooms will require more than one cleaning per day. Elevators: 1. Clean walls Daily 2. Clean floors Daily 3. Clean control panel Daily 4. Condition stainless steel walls Daily 5. Vacuum door tracks Daily Nuclear Medicine: 1. Sanitation of exam table and furniture Daily 2. Wipe all vertical and horizontal surfaces Daily 3. Remove trash and clean waste receptacle Daily 4. Spot wiping of walls Daily 5. Cleaning of vents Weekly 6. Clean floors Daily 7. Wall washing Monthly 8. Floor Stripping Semi-Yearly 9. Perform high dusting Daily 10. Floor burnishing Twice weekly Special Procedures: Special procedure rooms must be cleaned after each procedure. Employees shall be stationed in or near the areas to be accessible for these cleanings. There are approximately 12 of these rooms in which the procedure lasts about an hour. The contractor shall have 30 minutes to clean the room before the next procedure. These rooms shall be cleaned using the same procedures as those used to clean an operating room between cases. The contractor shall have experience in cleaning around very strong magnets that are contained in nuclear medicine rooms. Offices: 1. Remove trash & clean receptacle Twice Daily 2. Perform High Dusting Weekly 3. Wipe horizontal surfaces including bookcases Weekly 4. Spot wipe walls Daily 5. Clean floor ( vacuum or mop) Twice per Week 6. Carpet cleaning Twice per year The contractor shall clean the stairwells in the assigned areas as needed. They shall be maintained free of dust and litter at all times. The contractor shall also clean the entrances to the Clinical Research Center and one hundred feet outside beyond the entrances. The entrances to be cleaned are the main entrance to the CRC, the ambulance entrance to the CRC and the entrances leading to the courtyards. The contractor shall be responsible for emptying trash cans and cigarette butt containers. The contractor shall remove all litter, cigarette butts and trash from that area. The contractor shall keep the entrances clean by sweeping and pressure washing when necessary. The contractor shall maintain all walk-off mats and place and remove them when the weather warrants. The contractor shall also clean all of the enclosed courtyards of the Clinical Research Center in the same manner as the entrances. The contractor shall conduct all training contractor and government staff necessary to assure compliance with all federal, state, local, licensing and accrediting regulations. The contractor shall also perform all training deemed necessary by the Chief, HFCD. The contractor shall supply a fulltime training coordinator to train both Federal and contract employees. The contractor’s person shall be totally familiar with all pertinent JCAHO standards and all pertinent OSHA standards. This includes, but is not limited to, bloodborne pathogen precautions, airborne pathogen precautions, radioactive precautions, tuberculosis and multi drug resistant tuberculosis precautions, medical waste handling and use and handling of cleaning chemicals and machinery. The training coordinator shall plan all training for the Housekeeping & Fabric Care department. The contractor shall conduct a comprehensive floor program that will assure the cleanliness and gloss of all hard floor surfaces and the cleanliness of all carpeted areas. Stripping and burnishing will be included in the program and performed when necessary. The contractor will also conduct a comprehensive program of project cleaning to assure the cleanliness and appearance of the designated areas. Projects will include but not be limited to: Wall washing Furniture cleaning Interior glass cleaning Public areas, which are the main lobby, all waiting rooms and elevators will be policed by the contractor during normal business hours (8:00 AM to 5:00 PM Monday through Friday) and will be thoroughly cleaned during the off hours (6:00 PM until 7:00 AM). These areas must be free of dust, dirt and litter at all times. HFCD will supply all cleaning chemicals and equipment. The contractor will draw all chemicals from the HFCD supply room. All plastic trashcan liners and medical waste boxes will be supplied by HFCD. Complete maid carts with a locking cabinet will be supplied to the contractor. Mop heads will be supplied on a daily basis. Buffers, burnishers, floor machines, power wet/dry vacuums, carpet extractors and other necessary machinery will be assigned to the contractor by HFCD. HFCD retains the ownership of the equipment. The contractor shall be responsible for damage due to misuse or carelessness of its employees. The contractor shall supply a list of equipment needed in it proposal that would be used to clean the building. This list must include the type, description and number of the machines to be used by the contractor. The government shall supply uniforms for the contractor’s employees. The uniforms shall remain the property of the government. All of the contractor’s employees shall be in full uniform while on duty in the Clinical Center Complex. Each of the contractor’s employees shall receive an official NIH identification badge and will display the badge while on NIH property. The contractor shall conduct a quality assurance program that is compatible with the HFCD program and the program will be monitored by the Project Officer. The program must be computer based and have the ability to produce reports as requested. The contractor shall use the latest technology and labor saving devices. The government shall not pay for excess labor due to the contractor using outdated methods of cleaning. The contractor shall state in the technical proposal what technology shall be used to reduce labor. The supervisors of the contractor shall speak English fluently. A supervisor must be on site at all times to be able to translate to employees that may not speak English. The contractor shall supply a squad of cleaning personnel to be on call at all times from 6:00 AM until 6:00 PM Monday through Friday to handle any cleaning emergencies that may arise. The number of the employees comprising this squad shall be determined by the contractor and shall be stated in the contractor’s proposal. If an emergency arises, the user shall notify the contractor at the HFCD phone number. The contractor shall prepare schedules for cleaning of all assigned spaces. A copy of all current schedules shall be maintained with the Chief, HFCD and the contracting officer. In addition, the contractor shall negotiate written service agreements with all customers within 60 days of beginning work. Copies of these agreements shall be given to the contracting officer and the Chief, HFCD The contractor shall propose a communication system to be utilized to assure that prompt service is rendered. This system must deal specifically with providing service in the off hours (5:00 PM until 6:00 AM) to the nursing units for discharge cleaning. This system must include how customers may call to receive service. Contractors shall comply with all applicable federal and state government regulations regarding control of infectious diseases in health care facilities. Contractors should consult the specific regulations for further details, exemptions, documentation, and record keeping requirements. Specific examples include, but are not limited to: The contractor’s workers must have proof of rubella (measles) immunity. Workers born after 1956 working at least 20 hours each week who are newly retained as direct or contractual employees, or volunteers shall have documentation of receipt of either one dose of live measles virus vaccination or proof of immunity by blood test for antibody to rubella (Health-General Article, 19-307, Annotated code of Maryland). The contractor shall keep the measles vaccination or immunity status of each worker on file. Workers who have the potential for exposure to M. tuberculosis shall receive counseling, screening and evaluation. These workers include: persons who routinely have patient contact or who enter patient rooms or examination or treatment rooms; persons who are exposed to M. tuberculosis in a laboratory or morgue; or persons who work in a primate animal care setting. Workers who have positive purified protein derivative (PP) skin test results, PPD test conversions, or symptoms suggestive of TB should be identified, evaluated to rule out a diagnosis of active TB, and started on therapy or preventative therapy if indicated. The results of pre-placement and periodic PPD testing must be kept in a retrievable aggregate database, and PPD test conversion rates for each specific occupational group or each area of the hospital must be calculated at appropriate intervals (at least annually). To calculate PPD test conversion rates, the total number of previously PPD-negative workers tested in each area or group (i.e. the denominator) and the number of PPD test conversions among workers in each area or group (the numerator) must be obtained. These data must be reported to Hospital Epidemiology Service as requested. (Center for Disease Control and Prevention. Guidelines for preventing the transmission of Mycobacterium tuberculosis in health-care facilities, 1994. MMWR 1994:43 (No. RR 13): pp 1-132 Any contract employee physically working in building 10 which includes the CRC and the CC, whether or not they have occupational exposure to M. tuberculosis, must have a pre-placement PPD skin test and those with positive PPD tests must be evaluated as described above. Contract workers with patient contact must have a pre-placement evaluation for history of chickenpox (varicella) and/or receipt of varicella vaccine. The contractor shall keep the varicella history and/or vaccination status of each worker on file. The contractor’s staff shall follow all rules and regulations of the Clinical Center and NIH at all times while on NIH property. The contractor through the contractor’s onsite manager shall enforce the rules and regulations of the Clinical Center and NIH. In emergency situations (building floods, natural disasters, snow storms) the contractor’s employees shall be deemed emergency and will perform as emergency employees.
