Description

VA VISN 22 Network 22 Logistics Office intends to negotiate a Commercial (FAR Part 12) Firm Fixed Price contract and use of Simplified Acquisition Procedures (FAR 13) through a Request for Quote (RFQ) for Equipment Preventive Maintenance Services and Intervening Services for selected Audiometers, Middle Ear Analyzers and Sound Meters for the VA Greater Los Angeles Healthcare System (including the the Sepulveda campus (North Hills) and the Community Based Outpatient Clinics (CBOCs) in Los Angeles, Santa Barbara, Bakersfield, and Lompoc), the VA San Diego Healthcare System and VA Southern Nevada Healthcare System (Las Vegas) facilities. The solicitation document with incorporated provisions and clauses will be made available on or about 18 January 2005. The contract is being established for the purpose of servicing and maintaining the audiology equipment in local areas over a widespread geographical area encompassing the participating VA Healthcare Systems. The procurement will specifically provide for the contractor to satisfy the requirements of the Department of Health, Education and Welfare, National Fire Protection Agency (NFPA), Joint Commission of Accreditation of Healthcare Organizations (JCAHO), Association for the Advancement of Medical Instrumentation (AAMI) and the State of California and State of Nevada. The contract period is projected to be a base year from award of contract, with the Governments option to renew the contract for four (4) optional one (1) year periods. Copies of the solicitation will be made available upon written or faxed request. Interested persons may identify their interest and capability to respond to the requirement by submitting the quote. Qualified businesses wishing to receive a copy of the solicitation must submit their request in writing with complete company name, address, telephone number, FAX number, email address and a point of contact. All quotes with completely submitted required documentation received by 4:00 P.M. Pacific Time, 18 February 2005, will be considered by the Government. Inquiries will only be accepted in writing by US Mail or other hard-copy delivery agents.
