- The Indian Health Service, Division of Engineering Services is issuing a solicitation for a qualified applicant/contractor for a position as the Contracting Officer?s Quality Assurance Representative (COQAR) under a personnel services contract to provide quality assurance services and over sight on the construction of 57 staff quarters located on the Navajo Indian Reservation at Red Mesa, Arizona. This project is to provide housing units for staff members at a new medical facility which is now under construction. The term of this position are one calendar year with a one-year option. SPECIAL NOTICE: Living accommodations are not available at the sites. Government housing is not available and will not be provided. This is a remote location and the contractor will be responsible for making and providing his own living arrangements. Government Per Diem rates for the area maybe paid and will be based on a determination on applicability as specified in Government Regulations. The selected contractor shall provide all necessary service including transportation, office supplies and equipment to accomplish daily-filed observations and quality assurance checks to insure compliance with approved project plans and specifications. The contractor will be responsible for providing his own office equipment including Government compatible computers and soft ware, printers, fax machines, photo copy machines, office telephones, cell phones, cameras, office furnishings, and any and all required office supplies needed as none will be provided by the Government. Office type trailer will be available for office use, but may not be used as for living quarters. Duties include reviewing and preparing daily reports, assembling and maintaining record documents including preparing job progress reports with photographs, perform scheduling analysis, conduct submittal review and RFI reviews, conduct labor compliance interviews, review materials be used on the project for code and regulation compliance including Buy American Act compliance, monitor job safety program to assure strict compliance and interface with the onsite contractor?s superintendent and quality assurance representative to resolve minor problems as they develop. Contractor will be required to maintain constant communications with the Contracting Officer and the Government Project Manager, attend monthly progress meetings, participate in partnering meetings, review and make recommendations on monthly progress payments to the Contracting Officer and the Project Manager, review and make recommendations on modification including preparing cost proposals and developing negotiation position working with the Contracting Officer and the Project Manager, and any additional duties required of quality assurance representative for successful project oversight. To qualify for this position, your resume must show sufficient experience and/or education, knowledge, skills, and abilities to perform the duties of the specific position for which you are being considered. The qualifications submitted by each individual will be reviewed and evaluated to identify the most highly qualified individuals. When responding to this announcement, individuals should fully address their capability and qualifications with regard to each of the following evaluation factors. Individuals should submit their complete qualifications as they may be evaluated solely on their written responses to this announcement. For each factor you must prepare a two pages or less written statement that explains how you meet the specific factor. Failure to submit a response for each factor will negatively affect your eligibility and/or rating for this position. The Contracting Officer shall consider this information as well as information obtained from other sources when evaluating the applicant?s qualifications. The applicant is reminded that the Contracting Officer shall determine the relevance of similar experience information. Oral interviews may or may not be held at the option of the Government. Highest qualified applicants will be issued a request for proposal package, which will include request for pricing. Evaluation Factors: (1). PROFESSIONAL QUALIFICATIONS: Document professional registration and qualifications. Submit support documentation such as licenses, certifications, and college transcripts. Professional qualifications for design and construction of similar scale projects in civil, architectural, mechanical and electrical quality assurance and project related type experience. (2) EXPERIENCE WITH PROJECTS OF SIMILAR SIZE, SCOPE, and COMPLEXITY QUALITY CONTROL/QUALITY ASSURANCE/ SAFETY EXPERIENCE DURING CONSTRUCTION PHASE Applicant must show construction experience on similar projects of similar size and scope within the past 3 years including construction inspection and ability to interpret construction drawings and specifications, in order to serve as inspector of construction and contractors quality control on complex construction projects. Elaborate on projects that have been completed in the last 3 years. Provide reference that may be contacted. . (Failure to provide a minimum of 3 relevant references or the inability of the Government to reach at least 3 references, after making a reasonable effort to do so, may result in the applicant not being rated on the factor. Applicants that do not present sufficient information to permit complete evaluation by the Contracting Officer may be determined unacceptable and non responsive. If contracts cannot be verified because of lack of information, (i.e. POC or phone #) that reference shall be treated as if it had not been submitted) (3) QUALIFICATION AND EXPERIENCE OF OFFICE FUNCTIONS AND PREPARING REPORTS, ABILITY TO COMMUNICATE WITH OTHERS (e.g., ORALLY, and in WRITING) knowledge of computer systems in order to communicate with Contracting Officer and Project Manager and other agency personnel Experience and qualifications, knowledge of computer programs, project management software, and general office functions. (4) PAST PERFORMANCE WITH GOVERNMENT AND PRIVATE INDUSTRY CONTRACTS OF SIMILAR SIZE, SCOPE, AND COMPLEXITY: Experience on Federal Government projects, interaction with local jurisdictional authorities, and dealings with the Indian Community, specifically Navajo Tribe. (5) SALARY HISTORY: Provide verifiable salary history for quality assurance work performed in the past 3 years on similar type projects. Award will be made to the applicant that the Government determines can accomplish the requirements set forth in the solicitation in a manner most advantageous to the Government consistent with the selection criteria. The evaluation factors and other factors are approximately equal to 60% of the award factor. Price is a numerically weighted factor approximately equal to 40% of the award factor. Price is a significant factor and its importance as an evaluation factor will increase as the degree of equality of technical competence and approach between proposals increases. If the two or more highest rated technical proposals are considered to be essentially equal in terms of technical competence, price may become the determinative factor in making an award. The Government reserves the right to award a contract to other than the lowest priced offer after consideration of all factors.
