Description

VA VISN 22 Network 22 Logistics Office intends to negotiate multiple Commercial (FAR Part 12) Firm Fixed Price contracts and use of Simplified Acquisition Procedures (FAR 13) through a Request for Quote (RFQ) for Orthotic Fabrication and Repairs for the Loma Linda, Greater Los Angeles, Long Beach, San Diego and Southern Nevada VA Healthcare System facilities and areas located in the Southern California Counties of San Diego, Imperial, Los Angeles, Orange, Kern, Ventura, Santa Barbara, San Luis Obispo, San Bernardino and Riverside, the Nevada Counties of Clark, Esmerelda, Lincoln and Nye, the Arizona County of Mojave, and the Utah County of St. George . The solicitation document with incorporated provisions and clauses will be made available on or about 18 January 2005. This procurement is to obtain fabrication and repair of orthotics. The contracts are being established for the purpose of servicing the Veterans in local areas over a widespread geographical area encompassing the participating VA Healthcare Systems. The procurement will specifically provide for all contractors to have a full-time American Board Certified (ABC), or Board for Orthotist-Prosthetist Certification (BOC) or VA Qualified (VAQP) Prosthetist/Orthotist on staff at each of their locations in good standing. The contract period is projected to be a base year from award of contract, with the Governments option to renew the contract for two (2) optional one (1) year periods. Site visits may be required by the VA facility. Copies of the solicitation will be made available upon written or faxed request. Interested persons may identify their interest and capability to respond to the requirement by submitting the quote. Qualified businesses wishing to receive a copy of the solicitation must submit their request in writing with complete company name, address, telephone number, FAX number, email address and a point of contact. All quotes with completely submitted required documentation received by 4:00 P.M. Pacific Time, 18 February 2005, will be considered by the Government. Inquiries will only be accepted in writing by US Mail or other hard-copy delivery agents.
