- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation. Quotations are being requested; a written solicitation will not be issued. The simplified acquisition procedures of FAR Part 13 will be used in conjunction with the commercial item procedures in FAR Part 12. The solicitation is being issued as a Request for Quotation (RFQ). The solicitation number is RFQ 246-05-01854. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2001-27. The North American Industrial Classification Code (NAICS) is 811219 and the business size standard is $6.0 mil. Request contractor provide on-site services including all parts (except disposables) and labor required to keep the system operating at normal Original Equipment Manufacturer (OEM) performance specifications for a Jostra Bentley Model HL 20 Perfusion System consisting of the following modules: seven (7) Model 043600000 Blood Pumps; one (1) Model 043650000 Computerized Perfusion Controller; one (1) Model 043620071 Seven Position Console Base; one (1) Model 043660010 CRT; one (1) Model 043213105 Venous Line Clamp. All equipment is located at the VA Medical Center, Durham, North Carolina. One (1) Preventative Maintenance (PM) with complete calibration will be scheduled per year on a date mutually agreed between the government and contractor. All parts furnished must be OEM new or equivalent OEM re-conditioned. PM procedures to be performed in accordance with established OEM specifications as noted in the most current service manual. All services will be performed during the items 7:00 a.m. - 7:00 p.m. Monday through Friday, excluding federal holidays. Qualified OEM Field Service Engineers (FSE) should be on-site within 48 hours of contractor receiving request for service, excluding weekends and federal holidays. Contractor must have access to all OEM service manuals, service bulletins and updates, diagnostic software. Proposals must also include documentation and proof that FSEs servicing the equipment have obtained OEM model specific maintenance training. Contractor will provide loaner equipment at no additional charge if it is determined that equipment must be transported to an off-site facility for repairs. Offeror proposals are hereby being requested in response to the requirements under a firm-fixed price contract for a base period from date of award through September 30, 2005 with the option to extend services for up to four additional years through September 2009. Request offerors proposal list costs for each item separately and on a monthly basis per machine for the base and all option years. Proposals shall be submitted on offerors letterhead or bid form and shall contain the name, title, and signature of the person authorized to submit proposals on behalf of the contractor and include the information required in provisions 52.212-3, Offeror Representations and Certifications-Commercial Items (Jan 2005). Quotes shall be valid for 30 days from the offer due date. Award will be made to the Offeror who can meet the requirements listed above and is most advantageous to the Government. The Government intends to evaluate the information received and award without discussions. The following provisions and clauses apply to this solicitation: 52.212-1, Instructions to Offeror-Commercial Items, 52.212-4, Contract Terms and Conditions-Commercial Items, 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items, 52.233-2, Service of Protest, 852.233-70, Protest Content, 852.233-71, Alternative Protest Procedure. In accordance with FAR 52.204-7 award will be made only to a contractor/company that is registered in the Central Contractor Registration (CCR). Quotations and all required information must be received by 4:00 p.m. EST on January 26, 2005 and shall be submitted in writing to Doris Turner, Contracting Office (90C), VA Medical Center, CAS/VISN 6, Building #27, Averill Drive, Hampton, VA 23667. No fax or electronic offers will be accepted. Solicitation number RFQ 246-05-01854 shall be listed on the documents submitted for identification purposes. Only offers prepared in accordance with this solicitation shall be considered.
