Description

SPECIAL NEEDS (WHEELCHAIR/STRETCHER) PATIENT TRANSPORTATION SERVICES, for the Orlando VA Healthcare Center, including Nursing Home Unit, 5201 Raymond Street, Orlando, FL, and the Sanford Community Based Outreach Clinic, 1403 Medical Plaza Drive, Suite 109, Sanford FL. This is a synopsis for commercial services per the procedures at FAR 12, 13, and 15. Request for Proposals (RFP) Solicitation Number 673-33-05. Solicitation issue date 12 January 2005, proposal due date 9 February 2005, 4 p.m. eastern standard time. The North American Industry Classification System (NAICS) code is 485991, size standard $6M. This is a 100% small business set-aside. Contractor to provide all labor, materials, vehicles, equipment and supervision to provide transportation services for VA beneficiaries; wheelchair and stretcher patients. Period of performance: 1 April 2005 through 30 September 2005, with two 1-year option periods. Contractor must be licensed by the Orange County (Florida) Public Transportation Commission to provide these services.
