Description
NAICS: 561612 Provide a Security Guard for the Bangor VA Clinic, 304 Hancock Street, Suite 3B, Bangor, Maine. This contract will be awarded on or before 1 April 2005 and continue until 30 September 2005. Four option years will also be included to be renewed by 1 October of every year. The contractor will be responsible for training employees, testing, assigning competent workers, supervising, discipline of problem employees, payroll deductions, payment of income tax, social security (FICA), unemployment insurance and workers compensation, etc. The VA supervisor will be responsible for technical instruction to the employee and review of the employees work. Additional training may be required of the security guard to be conducted in an area other then Bangor. If additional travel is required the government will reimburse the employee for those additional travel costs up to the limit of the Joint Travel Regulations. Services shall be provided during the following work hours: (a) 8:00 a.m.- 4:30 p.m., Monday through Friday, excluding national holidays. (b) National Holidays: The ten holidays observed the Federal Government are New Years Day; Martin Luther King Day, President_s Day, Memorial Day, Independence Day, Labor Day, Columbus Day, Veterans_ Day, Thanksgiving and Christmas Day and any other day specifically declared by the President of the United States to be a National Holiday. Services to be provided are: a) The Security Guard is required to wear a uniform. b) The Security Guard shall provide physical protection of patients, employees and visitors. c) The Security Guard must respond to a variety of situations that occur in the clinic; i.e. disturbances, assaults, bomb threats, fires, etc. and shall prepare reports on situations involving the Security Guard_s action/interventions. d) The Security Guard shall provide assistance to patients, employees and visitors who may need directions or physical assistance. (e) The Security Guard shall detect possession of contraband; i.e. alcohol, drugs and weapons. Department of Labor Wage Determination No 1994-2241, Revision No. 26, dated 08/17/04 is made a part of this contract. For Classification purposes the Government is classifying the work to be accomplished and necessary training to fall within the Occupation Code of 27102, Guard II. Contractor shall provide a copy of the employee_s Measles/Mumps/Rubella status if born after 12/31/56. A copy of their immunization history and TB test shall be provided prior to commencement of employment at the Bangor VA Clinic. A TB test will be administered by Employee Health, located in Building 200, at the VAM&ROC, at no cost to the individual. This Immunization History form will be completed by the individual and faxed to the Contracting Officer at (207) 623-5762, prior to commencement of employment, for review by Employee Health. Contract employees must be present and performing the required services for the specified period to receive payment. Payment for any leave, holidays not worked or vacation time are the responsibility of the contractor. If holiday work is required, holiday pay for the employee shall be at the doubletime rate if the employee works within their scheduled tour of duty; payment shall be at time and a half if the employee works outside their regular tour of duty. By acceptance of this contract the contractor agrees to abide by all the rules and regulations of the Health Insurance Portability and Accountability Act of 1996 (HIPAA). All employees who have access to records designated as confidential and privileged under 38 U.S.C. 5705 will treat the findings, reviews and actions relating to quality assurance in a confidential manner. Each person accepted to work under contract for a period of six months or more, who will have access to the Department of Veterans Affairs_ computer systems shall be the subject of a background investigation and must receive a favorable adjudication from the VA Office of Security and Law Enforcement. The social security number and date of birth must be presented with the bid for each person presented to work under subject contract. The contractor will be responsible for the actions of those individuals they provide to perform the work for VA. The fee for this should be included in the hourly bid rate, the estimated fee for this is $200. Failure to comply with the contractor personnel security requirements may result in termination of the contract for default. Additional documentation to include with your bid is: Resume of proposed security guard; Business References; Background checks conducted on proposed security guard; Copy of the employees measles/mumps/Rubella if born after 12/31/56 Numbered Note 1 Applies.
