Description

Contractor to provide Certified Tumor Registry Services for the William S. Middleton Memorial Veterans Hospital, Madison, Wisconsin in accordance with specifications. Contract services are for the period from date of award through December 31, 2005 with one 12 month option to extend. Contractor will also provide services for a backlog that consists of the period of January 1, 2004 through contract award date. This procurement is 100% set-aside for Small Business. NAICS is 621999 with a small business size standard of $8.5 million in revenues. Quotations received will be evaluated on technical capability, price and past performance. Solicitation documents will be available January 18, 2005 with quotations due February 11, 2005. Offerors are reminded to complete all sections of the solicitation including the Offeror Representations and Certifications - Commercial Items. Requests for hard copy solicitation must be in writing to the Contracting Officer at the Great Lakes Acquisition Center and accompanied by a check for $5.00 made out to the Department of Veterans Affairs. No telephone requests will be accepted. Offerors are advised that they are responsible for obtaining amendments.
