Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This synopsis/solicitation is for the requirements cited in the line items contained within this synopsis. Award will be made only to contractors who have registered with the Central Contractor Registration at http://www.ccr.gov Award will be made only to contractors who have completed Representations and Certifications on-line at http://www.bpn.gov. The following provisions and/or clauses in the Federal Acquisition Regulation (FAR) apply to this acquisition to include any addenda to the provision/clause. This solicitation incorporates one or more clauses by reference, with the same force and effect as if they were given in full text. GENERAL: Scope of Work: Non-personal services to provide religious instruction and pastoral care in support of the Chapel OIC and overall chapel program. Duties include preparing and conducting ecumenical religious instructional classes, planning, coordinating and executing monthly activities, conducting religious retreats and providing professional development classes for coworkers who are volunteers. Services shall be performed weekly or on an as needed basis. Specifically, these services are: (1) Youth meetings lasting at least one hour in length . (2) Weekday bible study lasting at least one hour in length . (3) Individual counseling sessions lasting at least 30 minutes in length . (4) Coordination and planning meetings. (5) Retreats . (6) Special Events . The Contracting Officer or designated representative will coordinate the ordering and scheduling of the above activities with the contractor. The contractor will be given at least a 24 hour notice prior to being required to perform any of the above servi ces. The contractor shall not perform any work under this contract that exceeds the quantities of services required identified in the Contract Line Item Schedule unless prior approval is received from the Contracting Officer. a. Qualifications: Contractor and/or contractor employees performing services under this purchase order shall meet the following qualifications: (1) Possess a Bachelor's Degree with a major in religious studies, religion youth ministry, or equivalent. (2) Shall have a minimum of one-year experience in youth ministry. b. Evaluation Criteria: (1) Each offeror submitting a quotation for this solicitation will be required to meet with the Contracting Officer and the Soldier's Memorial Chapel Protestant Chaplain. Each offeror will be rated on his/her understanding of the requirements contained in this solicitation, their approach to the work required, and their experience and past performance in performing similar services. The Contracting Officer will contact the offeror and coordinate a meeting time. (2) Prices contained in the schedule. b.(1) is somewhat more important than b.(2). c. Special Requirements: (1) Contractor shall adhere to the highest moral and professional standards of dress and behavior typical to the standard of this industry at all times during the performance of work under this order. (2) The Contracting Officer's Technical Representative (COTR) for this purchase order is the Soldiers' Memorial Chapel Protestant Chaplain or Deputy Command Chaplain. (3) Contractor shall submit invoices (request for payment) to the Soldiers' Memorial Chapel Protestant Chaplain (See Ship To Address). Payment will be processed in accordance with Federal Acquisition Regulation (FAR) Clause 52.212-4 Contract Terms and C onditions-Commercial Items (OCT 2003) which is contained in this purchase order. (4) This is an indefinite-quantity purchase order for the services specified, and effective for the period stated, in the Schedule. The quantities of services specified in the Schedule are estimates only and are not purchased by this order. The quantitie s specified for each item in the schedule shall not be exceeded unless prior approval is received from the Contracting Officer. (5) Contractor shall perform the requirements of this purchase order only as authorized by verbal or written instruction issued by the Contracting Officer or designated representative. The Contractor shall furnish to the Government, when and if ordered, the services specified in the Schedule and Statement of Work (SOW). d. Condition of Award: The Government will perform a background check on the successful offeror and offeror must pass the background check in order to receive the award of a purchase order for this requirement.
