Description

Provide all personnel and equipment necessary to supply Tc99m Sestamibi for myocardial imaging perfusion imaging procedure; for the Department of Veterans Affairs John D. Dingell Medical Center, Acquisition Management (001R-A), 4646 John R. Street, Detroit, MI, 48201. All items under the terms of this contract shall be handled by a registered radiopharmacist. Prior to acceptance of any items, the contractor shall provide documentation of the current licensure/registration of all personnel who will be performing under this contract. The NAICS code for this procurement is 446110. Projected period of service is from date of award through February 29, 2006, with the possibility of three pre-priced option years. Requests for Quotes (RFQ) are now available on this website. Requests for quotes may be requested in writing to the above address or via facsimile at 313-576-1077. Telephone or internet requests will not be accepted. Quotes are due by 4:00 pm local time, Friday, February 18, 2005. Quotes received after due date will not be considered. Interested parties must be authorized (in writing) by Dupont (and submit with proposal) to provide Sestamibi Tc99m radiopharmaceuticals. Contractors are responsible to verity documents have been received in the contract office.
