- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This solicitation is issued as a request for quote (RFQ). Submit written offers (oral offers will not be accepted), on RFQ F2U31250210100. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2001-14 and Defense Acquisition Circular 91-13. This procurement is being issued as unrestricted. The North American Industry Classification System (NAICS) Code is 562991 and the size standard of $6,000,000.00 applies to this procurement. The requirement is for 320 portable toilets with services during 19-20 Mar 05 Luke AFB Air Show. 1. CONTRACTOR QUALIFICATIONS: The contractor shall be regularly engaged in the service of providing portable toilets, and shall have suitable equipment for adequately delivering, servicing, and maintaining portable toilets as required in paragraph 5 of this SOW. 2. EQUIPMENT REQUIREMENTS: The toilets shall be self contained, designed to be sanitary and to include a hand sanitizer dispenser. They shall be constructed of fiberglass or plastic with round corners free of cracks or seams that could contain contaminates. The units shall be vented to minimize orders and shall have a latch to hold the door closed. 3. CHEMICALS: The contractor shall furnish Material Safety Data Sheets (MSDS) of compounds proposed for the use in cleaning and disinfecting process and shall certify that all chemicals to be used in performance of the contract work will be non-toxic and non-corrosive. This information shall be presented to the contracting officer at the time of pre-performance conference in accordance with FAR Clause 52.223-3. The CO or Quality Assurance Evaluator (QAE) will provide MSDS?s to 56 CEV/CEV for review prior to the contractor bringing products on base. The contractor must inventory all HAZMAT, including but not limited to: solvents, paints, degreasers, greases, Ozone Depleting Substances, and oils, brought on base. A HAZMAT inventory must be maintained when used containing the nomenclature, manufacturer, part number, quantity, storage location, and the process in which the material is used, (see attachement 1). All HAZMAT stored at Luke AFB will be managed in accordance with federal, state, local, and Air Force regulations. Identified HAZMAT will be managed and tracked in the installation?s approved HAZMAT tracking system. If a HAZMAT is required tracked in tracking system, contractor will completed require documents to support installation requirements. Contractor will report HAZMAT inventories/usage to the CO or QAE not later than 10 business days after completion of contract. QAE?s will provide inventory/usage to the Hazardous Material Management Office (Bldg 927). 4.DELIVERY REQUIREMENTS: The contractor shall place in designated areas 300 regular portable toilets and 20 handicap toilets. Set up shall begin at contractors discretion on Friday, 18 March 2005 and be completed not later than 4:00 p.m. Friday, 18 March 2005. Staging area is available for contractors use Monday, 14 March 2005 through Thursday, 17 March 2005. 5. SERVICE REQUIREMENTS: The contractor shall provide stand by service with equipment and employees for pumping, sweeping, scrubbing with a disinfectant, filling the reservoir with approved chemical, refilling hand sanitizer dispenser and providing toilet paper to last until next service. Services shall be available on the 19th and 20th March 2005 during air show hours, 8:00 a.m. to 4:00 p.m. Payment for stand-by service shall start after the contractor arrives on Luke AFB and reports to the contract monitor or on site coordinator. All waste generated by the service/cleaning shall be hauled to off base location. 6. PICK-UP: The contractor shall remove all units from the show area (flight line) off base or to staging area not later than 10:00 p.m. on 20 March 2005. All staged units shall be removed off base not later than 12:00 p.m. on 23 March 2005. 7. REQUIRED IDENTIFICATION. The contractor shall ensure that all vehicles used in the course of this contract are properly registered, and comply with Luke AFB and AZ state insurance requirements. Contractor shall obtain vehicle and employee passes from Pass and ID Section of Security Forces, 623-856-6521, located at building 1200, cross streets Glendale Ave and El Mirage Rd ( east side El Mirage road, and north of Glendale Ave), prior to being allowed base access. The contractor shall submit the following to the CO for each employee requiring access to Luke AFB: full name, social security number, date of birth, height, eye color, and body weight. This list shall be provided to the CO prior to the contract start date. The provision at FAR 52.212-1, Instructions to Offerors--Commercial Items, applies to this acquisition. The Government will award a purchase order resulting from this solicitation to the responsible offeror whose offer, conforming to the solicitation, will be the most advantageous to the Government, price and other factors considered. The clause at FAR 52.212-3, Offer Representation and Certifications ? Commercial Items applies. Contractors shall submit the representations and certifications with their proposal. The clause at FAR 52.212-4,Contract Terms and Conditions-Commercial Items, applies to this acquisition. The clause at FAR 52.212-5 (Dev), Contract Terms and Conditions Required To Implement Statutes or Executive Orders--Commercial Items, applies to this solicitation.
