Description
This is a combined synopsis/solicitation for commercial services and is prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This solicitation uses Simplified Acquisition Pr ocedures (SAP) authorized in FAR 13.5 and is expected to result in the award of one firm-fixed unit price contract using commercial contract standard format 1449. This announcement constitutes the only solicitation; quotations are being requested, and a w ritten solicitation will not be issued. W91YTZ-05-T-0062 is a request for quotation (RFQ); its document and incorporated provisions and clauses are those in effect through the Federal Acquisition Circular 2001-23 and 2001-19 Update. This procurement is fu ll and open competition. NAICS CODE 621111. Size Standard: $8.5M. Non-Personal Services required for a part-time (10 hours per week), Pediatric Urologist to support the Department of Surgery, Urology Services, WRAMC. The Government will evaluate the quality of professional and administrative services provided and retain control over the medical and professional aspects of services rendered (e.g., professional judgments, diagnosis for specific medical treatment). The government-designated official is the Chief, Urology Services, Walter Reed Army Medical Center (WRAMC), o r his designated representative. Performance Work Statement is incorporated into and made part of this solicitation and any resulting contract for a period of 14 Feb 2005 to 30 Sep 2005. Credentialing will be thru WRAMC. The contract provider shall posses s the following credentials as a minimum: a current license to practice as an urologist in any state or the District of Columbia, board certification in urology with fellowship training in pediatric urology, have a minimum of two years experience at the s taff level with experience in an academic environment teaching residents and medical students. SPECIFIC DUTIES/TASKS. Provide full clinical evaluation on patients in an outpatient care setting, on site at WRAMC. WRAMC credentialing package is required 7 days after award. Immunization information is require at time of initial request for clinical privileges and annually thereafter. Information with points of contact and further instructions will be provided upon award. Performance will be scheduled during standard duty hours, excluding legal public holidays and other times the clinic is authorized to be closed by the MTF Commander. Duty hours include a one (1) hour unpaid lunch period. Hours worked do not includ e travel to and from the Medical Treatment Facility [MTF].Health Requirements. Contractor/contract employee(s) performing services shall comply with the health and immunization requirements as stated in the Addendum to FAR clause 52.212-4, entitled Health and Immunization Requirements, at the time of initial request for clinical privileges and annually thereafter. Replacement contract employee(s) shall be required to meet all of the same requirements as the primary provider, to include annual updates. The expense for all physical examinations to comply with the health requirements shall be borne by the contractor at no additional cost to the government. The contractor shall submit evidence, if required Immunization. The following provisions in their lates t editions apply to this solicitation: (1)FAR 52.212-1, Instructions to Offerors with the addendum that: The offer submitted shall include: (1) Proposed prices; (2; completed copy of FAR 52.212-3,Offeror Representations and Certifications-Commercial Items ; and (3) Past Performance, minimum one reference for similar scope of work, with whom past performance can be verified to include contact name, telephone number, email address and contract number and dollar value. Offerors may submit any other informatio n regarding specific contracts that they deem relevant to the evaluation of your past performance. (2)FAR 52.212-2, Evaluation, with t he following addendum for the basis of award: Award will be made to the quote whose response, represents the best overall value to the government. The Government reserves the right to award to other than the low offeror, price, technical and past performa nce will be considered. The Government may consider information available from other sources. (3) FAR 52.212-3 Alt I, Offerors Representations and Certifications; a completed copy of which shall be submitted with the offer. (4)FAR 52.212-4, Contract Terms and Conditions, with requirement for HIPAA (Privacy Act) and Immunization clauses. The contract provider is responsible for insuring that Occupational Health Services are provided for its employees in accordance with Federa l, OSHA and MTF requirements, including, but not limited to: CDC Infection Control Guidelines in Health Care Personnel, 1998; 29 CFR 1910.10.1030, Blood borne Pathogen Standard, and 29 CFR 1910.134, Respiratory. (5)FAR 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders, specifically.
