Description
This is a combined synopsis/solicitation for commercial items identified below, prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice and incorporates provisions and clauses that are in effect through Federal Acquisition Regulation Circular 2001-26 and 2001-27 dated January 1, 2005. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number is 516-032-05 and being issued as a Request for Quotation (RFQ). This procurement shall be solicited as a 100% set-aside for small business. The NAICS Code for this procurement is 421450 and the small business size standard is considered 100 employees, Numbered Note No. 1 applies to this solicitation. The VA Medical Center, Bay Pines, FL, is seeking a contractor to provide a brand name or equal for the following equipment: One Computerized Vascular Lab System, Parks Flo-Lab Model 2100 with VIP System. The system contains the following salient characteristics: Pentium IV 1.8 GHz, 128 MB Ram, 20 GB hard drive, 56 K Modem, 1.44 floppy drive, 36X CD-ROM, 250 meg Zip Drive; Intel pro 100 Network Card, Windows XP Operating system; 17? flat panel color monitor, Hewlett Packard 900C series printer; Vip Software with special features for 2100 only; two frequency, bi-directional Dopplers (4&8 MHz); forward and reverse Doppler Flow indicators; dual channel PPG; dual channel PVR; automatic cuff inflator, bedside remote control, Doppler volume and mute, navigation arrows, auto scale, and data storage, single/dual strip chart recorder, mobile cabinet with storage space, complete cuff package, 2 CC17, 2 SC12ex, 6 SC12, 4 SC10, 2 TMC7, 2 SC5, 2 DC2.5, 2 DC1.9, 2 PC 2.5, 1 PC1.9, One full on-site training is included, all software upgrades are free for life of the unit; computer is fully net-workable and interfaces with the PAC system; system is not computer dependent, that is, all exams can still be performed if the computer should fail; manual analog input to override computer preference; Doppler spectrum analysis; multi-media capability; system includes built-in protection using a ?medical grade? (UL 544 spec) isolation transformer for added patient and technician safety; Doppler probes can be ?cold gas sterilized? and used in a sterile field, Multi-Port Cuff Selector, DICOM for Flo-Lab, and Touch Screen Monitor. Delivery terms shall be FOB origin to the VA Medical Center, Warehouse, Bldg 100, 10000 Bay Pines Blvd, St. Petersburg, FL 33708. Proposals shall be submitted on an ?all or none basis.? Partial proposals shall be rejected. If submitting on a brand name equal basis, sufficient technical data must be included for the government?s evaluation. Offerors responding to this announcement shall provide all information contained in the Federal Acquisition Regulation (FAR) Clause 52.212-1, Instructions to Offeror?Commercial Items. Offerors shall include a completed copy of provision FAR 52.212-3, Offeror Representations and Certification?Commercial Items and VAAR 852.219-70 Veteran-Owned Business. In addition, incorporated herein by reference are the following VA Acquisition Regulations (VAAR) clauses and provisions: 852.211-70 Service Data Manuals, 852.211-71, Guarantee, 852.211.77 Brand Name or Equal, 852.270-4 Commercial Advertising.
