- NOTICE OF INTENT TO BUNDLE The VA National Acquisition Center will issue a request for proposal (RFP) for the Department of Veterans Affairs (VA), Department of Defense (DOD), Indian Health Services (IHS), and the Bureau of Prisons (BOP) requirements for Lactulose Syrup. The following items are Set-Aside for Small Business participation only: 1. Lactulose 10gm/15ml Syrup, package size 240ml estimated usage - 138,195 units 2. Lactulose 10gm/15ml Syrup, package size 480ml estimated usage - 214,779 units 3. Lactulose 10gm/15ml Syrup, package size 960ml estimated usage - 35,466 units The following line items will be Unrestricted: 4. Lactulose 10gm/15ml Syrup, package size 15ml UD estimated usage - 51,668 units. 5. Lactulose 10gm/15ml Syrup, package size 30ml UD estimated Usage - 10,209 units. The estimated quantities listed above are for VA, IHS and BOP. The contract period will be for one year plus four one-year options to extend. The contract items will be distributed through the VA and DOD Pharmaceutical Prime Vendors (PPV). Offerors must provide bar code labeling which includes at a minimum the NDC number. Offerors must state the exact name (name that appears on label) by the drug will be supplied and have a unique NDC number. If the offeror is not a manufacturer, the offeror must submit either (1) a letter of commitment or an agreement from the manufacturer which will assure the offeror of a source of supply sufficient to satisfy the Government's requirements for the contract period OR (2) evidence that the offeror will have an uninterrupted source of supply from which to Satisfy the Government's requirements for the contract period. Glass bottles are not acceptable. The solicitation will be available for downloading, with the issue date of on or about 02/04/05, from the VA Office of Acquisition and Materiel Management's (OA&MM) internet website at http://www.bos.oamm.va.gov/. Offers will be due by 4:00 P.M. Central Standard Time on 2/25/05.
