65- Medical Supplies-Potential Sources Sought Military Treatment Facilities (Army, Navy, Air Force, Marine Corps, Coast Guard) TRICARE Pacific Region 12 Patient Assist Device-Crutches Wood and Aluminum Point of Contact: Susan Hwang, Team Leader/Logistician, Tri-Service Regional Business Office, TRICARE Pacific Region 12, Honolulu, Hawaii. Telephone:(808) 433-3686. Potential sources sought to participate in the regional standardization of Crutches Wood an d Aluminum. TRICARE Pacific Region 12, a military integrated delivery network (IDN), comprised of Army, Navy, Air Force, Marine Corps and Coast Guard medical treatment facilities in Hawaii, Guam, Korea, Japan and Okinawa are in the process of standardizing Crutches Wo od and Aluminum. If your company holds or is in the process of obtaining a Distribution and Pricing Agreement (DAPA) with the Defense Supply Center Philadelphia (DSCP) for Crutches Wood and Aluminum, your company is eligible to participate in the standardization process. T his is in pursuant to the DAPA clause by which DAPA holders agree to the standardization process. Please respond by e-mail: susan.hwang@amedd.army.mil and provide your (1) company name, (2) address, (3) point of contact, (4) telephone number, (5) fax number, (6) e-mail address, and (7) answers to the below listed questions no later than February 25, 20 05. For questions, please call (808) 433-368 6. 1. What is your DAPA number for Crutches? 2. Do you have a distribution agreement with our Prime Vendor Cardinal? 3. Can you provide comprehensive educational resources, tools, and literature for the clinical application of Crutches? 4. Can you provide service to the entire region to include Military Treatment Facilities in Hawaii, Japan, Okinawa, Guam, and Korea? 5. Can you provide on-site in servicing and training to our Military Treatment Facilities in Hawaii, Japan, Okinawa, Guam, and Korea? 6. Please provide a history of backorders or recalls for this product group. 7. Describe the professional industrial production standards utilized in the manufacturing of the Crutches. 8. Under what brand name does your company manufacture or distribute your products? 9. Describe your return goods policy. 10. What type of customer service support do you offer? (i.e. 800 number, product representative, web support) 11. Is all of your products latex free? 12. How is your product safety tested and approved as a crutch? 13. Can you provide both wood and aluminum crutches. 14. Describe the material used for the wood crutch. 15. The arm pit rest portion of the wood crutch is covered with a cushioning material. Describe the composition of the material. 16. What type of cover is on the ground tips of the crutches? 17. Can you supply both adult and pediatric sizes of crutches? 18. Can you supply both traditional configuration and forearm support cradle with hand grip aluminum crutches? 19. Do you provide additional screws, washers and wing nut assemblies if required? 20. Are your aluminum crutches manufactured with only a telescoping height adjustment mechanism?
