- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 01-27. The North American Industry Classification System Code is 612512, Diagnostic Imaging. The small business size standard is 11.5 million average annual receipts for the past three years. This procurement is unrestricted. Offerors are encouraged to submit quotes based on the estimated number of examinations listed below for each location. Award may be made to one vendor providing services at all three locations or separate contracts maybe awarded to providers in each location. Proposed rates shall include all costs associated with providing the following services. Offerors are encouraged to submit the best possible initial offer. The government reserves the right to make award based on lowest price to offerors meeting the minimum requirements and satisfactory past performance ratings, without holding discussions. Request For Quotes (RFQ) 515-18-05 to provide all facilities, equipment, personnel, and supplies necessary to perform off-site mammography examinations and related ultrasound exams as follows for: I. VAMC, Battle Creek, MI and within the Battle Creek, MI metropolitan area: 101. Mammography, Bilateral (76091) Est. 2 each, with computer aided detection (76091 & 76082) Est. 20 each. 102. Mammography, Unilateral, (76090) Est. 2 each, with computer aided detection (76090 & 76082) Est. 7 each. 103. Screening mammography, bilateral (76092) Est. 2 each, with computer aided detection (76092 & 76083) Est. 30 each. 104. ULTRASOUND BREAST Unilateral Est. 7 each (If required). 105. ULTRASOUND BREAST Bilateral Est. 3 each (If required). II. VAMC Battle Creek, M and within the Kalamazoo, MI metropolitan area: 101. Mammography, Bilateral (76091) Est. 25 each, with computer aided detection (76091 & 76082) Est. 20 each. 102. Mammography, Unilateral, (76090) Est. 10 each, with computer aided detection (76090 & 76082) Est. 7 each. 103. Screening mammography, bilateral (76092) Est. 100 each, with computer aided detection (76092 & 76083) Est. 30 each. 104. ULTRASOUND BREAST Unilateral Est. 10 each (If required) and 105. ULTRASOUND BREAST Bilateral Est. 5 each (If required). The contract or contract(s) will be for a base year with four consecutive, pre-priced option years as specified in C.5 below. C.1.0. SCOPE: Provide mammography services at one, two, or all three locations specified below. C.1.1. Contractor will provide all supplies, equipment, personnel, and services necessary for mammography examinations. Each of the Contractor(s) facilities shall be located within the area of Battle Creek, Grand Rapids, and Kalamazoo, Michigan and/or their adjoining townships. C.1.2. Appointments for examinations shall only be made by authorized VA personnel and shall be available within 30 days of request. C.1.3. Typewritten reports shall be returned to VA within 48 hours after examination is completed. Facsimile or overnight mail shall be used to meet this deadline. C.1.4. The contractor shall provide original films to the Ann Arbor VA upon request, if follow up care is needed. C.1.5. The interpreting radiologist shall, if indicated, request additional views consisting of unilateral and bilateral breast ultrasound examinations. The contractor shall contact the VA for a doctor_s order to perform this additional testing. C.2.0. CERTIFICATIONS: C.2.1. Certifications shall be provided prior to contract award. C.2.2. Radiologist(s) shall be board certified in Radiology and maintain proficiency in mammography interpretations, a minimum 50 interpretations per week. Technologist(s) shall possess current certification by the American Registry of Radiologic Technologists, preferably in mammography. C.2.3. Mammography equipment shall be certified annually by an American College of Radiology, Board Certified Physicist and licensed annually by the State of Michigan. C.2.4. Payment for services rendered shall not be made until reports are received at the VA. C.4. SPECIAL CONTRACT REQUIREMENTS The contractor agrees, in accordance with the terms and conditions stated herein, to furnish for the Department of Veterans Affairs Medical Center, Battle Creek, Michigan, the services specified in the Statement of Work at the prices specified in the Pricing Schedule. C.4.1. RESOURCES C.4.1.1. The services specified in the Statement of Work may be changed by written modification to this contract. The modification will be prepared by the VA Contracting Officer and, prior to becoming effective, shall be approved by the Under Secretary for Health or his/her designee. C.4.1.2. The service to be performed by the contractor will be under the direction of the Chief, Radiology Service or his/her designee. C.5. TERM OF CONTRACT The contract(s) will be effective from April 1, 2005, through March 31, 2006, and subject to award of four, one year, option periods as determined by the Government. The contract(s) is subject to the availability of appropriated funds. The contractor(s) shall perform no services, after September 30, 2005, or September 30th of any subsequently exercised option period, until the Contracting Officer authorizes such services in writing. C.6. QUALIFICATIONS Contractor personnel furnishing services under this contract shall be licensed and registered to practice in the State of Michigan. Individual qualifications shall also be subject to review, by the Chief of Radiology Service, or his/her designee. C.7. MEDICAL RECORDS: Clinical or other medical records of VA beneficiaries treated by the contractor will be forwarded to the VA Medical Center, Battle Creek, Michigan. C.8. MONITORING PROCEDURES C.8.1. A description of the monitoring procedures used by the VA Medical Center to ensure contract compliance will be established as follows: C.8.2. Appointment will be made with the vendor and entered into the VAMC Veterans Health Information Systems & Technology Architecture (VISTA) by a designated VA employee. C.8.3. Upon receipt of verified reports, an entry will be made in the Radiology portion of the VISTA program from which computerized tracking will be generated, matched with the vendor's billing, and verified by the Chief, Radiology Service or his/her designee. C.8.4. The contractor shall submit individual invoices, which will include the patients name, the date of examination, CPT coded examination and the fee charged. Billing shall be submitted no later than the five days following the date service was provided. C.8.5. Any incidents of contractor noncompliance as evidenced by the above monitoring procedures will be forwarded immediately to the contracting officer. The Government may evaluate the quality of professional and administrative services provided, but retains no control over the medical, professional aspects of services rendered (e.g., professional judgments, diagnosis for specific medical treatment), in accordance with FAR 37.401(b). C.9. PERSONNEL POLICY C.9.1. The contractor shall be responsible for protecting the personnel furnishing services under this contract. To carry out this responsibility, the contractor shall provide Worker's Compensation, Professional Liability Insurance, Health Examinations, Income Tax Withholdings, and Social Security Payments. The parties agree that such personnel shall not be considered VA employees for any purpose and shall be considered employees of the contractor. C.9.2. LIABILITY: Worker's Compensation and Employer's Liability: Contractors are required to comply with applicable Federal and State workers' compensation and occupational disease statues. If occupational diseases are not compensable under those statutes, they shall be covered under the employer's liability section of the insurance policy. Employer's liability coverage of at least $1,000,000 shall be required. C.9.3. GENERAL LIABILITY: The contracting officer shall require bodily injury liability insurance coverage written on the comprehensive form policy of at least $500,000 per occurrence. C.9.4. The above referenced insurance coverage minimums are in reference to FAR Clause, 852.237 07 listed in Addendum to FAR 52.212-4, of this document. C.9.5. Certificate of Insurance shall be received before start of work. C.10. CONTRACT WORK HOURS: C.10.1. The services covered by this contract shall be furnished by the contractor as defined herein: The contractor shall not be required to furnish such services, except in case of an emergency on a national holiday or during non working hours as described below. C.10.2. The following terms as used herein have the following meaning: C.10.2.1. Workdays - Monday through Friday, 8:00 a.m. to 4:30 p.m. C.10.2.2. Non-Working Hours: 4:30 p.m. to 8:00 a.m. Monday through Friday, all Saturdays and Sundays. C.10.3. National Holidays - the ten holidays observed by the Federal government are New Years Day, Martin Luther King Day, President's Day, Memorial Day, Independence Day, Labor Day, Columbus Day, Veterans Day, Thanksgiving Day, and Christmas Day. C.11.0 JCAHO COMPLIANCE STANDARDS: The contract provider(s) shall continuously meet or exceed all Joint Commission on Accreditation of Healthcare Organization (JCAHO) Hospital standards. The patient population treated by the VA Medical Center, Battle Creek, Michigan, consists of veterans with varying physical, psychiatric, and psychological needs. Their conditions are, in many cases, directly related to the individual_s service to this country. All applicable VAMC policies and procedures shall be followed by the contractor. C.11.1. Patient Rights: C.11.1.1. All eligible veterans with assessed needs will be referred for mammogram services through the computerized patient record system (CPRS). Contract provider(s) will initiate direct communication with the patient as clinically indicated. Care provided will respect and integrate the patient_s beliefs, values and cultural influences. The contractor shall involve the patient in decisions by keeping him/her fully informed about the diagnosis, plan of care and treatment goals, risks and benefits of proposed treatment, and prognosis. C.11.1.2. Protection of all patient rights is of highest priority. VAMC staff will provide all patients a copy of the Patients Rights handbook on entry to care. Patient privacy and confidentiality will be maintained at all times. The contracted physician is required to review the Patient Rights handbook, and follow the standard of care. No patients will be subject to any research activity without his fully informed consent. The contractor shall abide by the VAMC Code of Ethics, which addresses marketing, admission/transfer and referral processes, and billing issues. Patient privacy and confidentiality shall be maintained at all times. All patient complaints which cannot be resolved by the contractor shall be referred to the VAMC Patient Representative. C.11.1.3. VA _ The contractor and patient care services provided shall be included in VAMC data collection activities related to patient compliments, complaints and satisfaction. C.11.2. Patient Safety: Patient Safety shall be a primary focus of the contractor. Every effort shall be made by the contractor to prevent falls, and patient injury caused by acts of commission or omission in the delivery of care. Contractor shall follow all Patient Safety Standards. C.11.3. Patient Assessment: All care provided shall be based on assessed patient needs. Contractor findings will be recorded and forwarded to the VA for inclusion in the patients computerized medical record. All events related to patient injury, medication errors, and other breeches of patient safety will be documented utilizing the contractor_s format and reported to the COTR C.11.4. Care of Patients: All care shall be provided by licensed, qualified, competent, and appropriately privileged individuals. Medication control, security and safety will be maintained at all times. No samples will be accepted from vendors or given to patients. Medications shall only be dispensed, prepared, and administered to patients by appropriately licensed and competent individuals. C.11.5. Patient Education: Education provided by the contractor will be interactive with the patient, and will be aimed at disease management and prevention and quality of life. C.11.6. Continuum of Care: The contractor shall be responsible for, obtaining additional views consisting of unilateral and bilateral breast ultrasound examinations, as appropriate. C.11.7. Performance Improvement: The contractor shall have a Performance Improvement (PI) program. Monitoring for continuous compliance with Joint Commission standards through established data collection mechanisms is recommended. Aggregate and/or practitioner-specific measurement and outcome data shall be maintained Measures related to contract compliance will be determined by the Contracting Officer_s Technical Representative (COTR). High priority shall be given by the contractor to the prevention of risk for the patient. The contractor shall implement process-related internal controls to prevent risk to the veteran. C.11.8. Environment of Care: The contractor shall maintain a safe environment at all times, and follow all safety and emergency preparedness policies. Compliance with all safety management plans will be continually demonstrated. C.11.9. Human Resources: The contract provider(s) shall be a board-certified in the modality(s) specified and the contractor will continually monitor performance. Outcomes from monitoring data, peer review and the patient compliment and complaint process will be kept on file and be incorporated into the re-privileging process. The contractor shall maintain a culture that promotes self-development and learning, which is designed to maintain or enhance required competencies. C.11.10. Orientation: Minimum orientation requirements include the following: VAMC Mission/Vision/Values. All CME requirements shall be met to maintain license. C.11.11. Information Management: The contract provider(s) shall meet all time frames for results reporting. C.11.12. Leadership/Management: The contract provider(s) shall effectively manage daily operations, information and support systems, and conserve physical and financial assets while meeting the mission of the VAMC. C.11.13. Medical Staff: The contract provider shall practice within a framework that is clinically relevant and scientifically valid. Provision of care shall be based on clinical standards of practice, and scientifically based clinical practice guidelines. C.12. MEDICAL CENTER MISSION, VISION, AND VALUES STATEMENTS The patient population treated by the Department of Veterans Affairs consists of veterans with varying physical, psychiatric, and psychological needs. Their conditions are, in many cases, directly related to the individual_s service to this country. Contractor employees shall provide compassionate care with respect for the special needs of the veteran population served, and show professional courtesy toward the VA support staff as an integrated part of the medical center_s mission, vision and values as stated below. Mission: The mission of the Battle Creek VA Medical Center, including Community Based Outpatient Clinics, is to improve the health of those veterans served by providing primary care, comprehensive psychiatric care, specialty care, extended care and related social support services as a member of an integrated healthcare delivery system. Vision: To become a recognized leader in primary care, psychiatric and geriatric health care, education, clinical research, and human relations, through effective teamwork. We will foster an atmosphere of mutual trust, respect, and pride by those giving and receiving health care. We will continuously improve the quality of services provided. Values: Trust; Respect; Commitment; Compassion; Excellence The provision at 52.212-1, Instructions to Offerors _ Commercial (JAN 2005), applies to this acquisition. The provision at 52.212-2, Evaluation - Commercial Items (JAN 1999), applies to this procurement. Offerors are required to submit the following documentation with proposals: Minimum Requirements: (1) Copy of full and unrestricted license to practice medicine in the State, Territory or Commonwealth of the United States or the District of Columbia; where the work will be performed and (2) Copy of board certification or board eligible in Radiology. Documentation of the minimum requirement stated above shall be submitted with offers. Evaluation criteria included below are, in descending order of importance, (1) Past Performance, (include at least three references for the same or similar work with proposal), (2) Competency, (submit qualifications, competency assessments, and other relevant documentation), and (3) Price. Past Performance and Competency are significantly more important than Price. Offerors are required to completed the provision at 52.212-3, Offeror Representations and Certifications - Commercial Items, (JAN 2005), with its offer.
