Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation. Quotes are requested. The Federal Acquisition Regulations (FAR) subpart 13.5 entitled “Test Program for Certain Commercial Items” applies to this solicitation. Solicitation number N33260-05-001 is applicable and is issued as a Request for Quote (RFQ). The solicitation and incorporated provisions and clauses are those in effect through Federal Acquisition 2001-03. This requirement is 100% set-aside for Native American and HUB-Zone Small Businesses with a NAICS Code of 562111. ITEM DESCRIPTION: Work consists of furnishing all labor, services, supervision, materials, supplies, equipment, transportation, and other incidentals necessary to provide refuse collection and disposal services at the schools and agency at Western Navajo Agency. Contractor will provide covers for the trash bins. Contractor will propose a firm fixed price yearly amount for one year and four option years. The first year is estimated to be 31 weeks and the period of performance will be from March 1, 2005 to September 30, 2005. The four option years will for one year, 52 weeks, from October 1 to September 30. Line Item 0001 Dennehotso. Line Item 0001AA – Building 201, 1 medium size bin, twice a week. Line Item 0001AB – Building 250, 1 small, 1 medium, and 1 large bin, twice a week. Line Item 0001AC – Quarters, 24 quarters with 18 currently occupied with 18 small bins, twice a week. Line Item 0002 Tonalea Line Item 0002AA – Quarters, 6 quarters with 4 occupied, 1 mid-size bin, twice a week (Tuesdays and Thursdays). Line Item 0002AB – School Building, 3 5X9 green bins located between buildings 887 and 869, twice a week (Tuesdays and Thursdays). Line Item 0003 Tuba City Boarding School. Line Item 0003AA – Tuba City Boarding School Quarters, 97 quarters, 40 gallon container required per quarter, twice a week (Monday and Thursday). Line Item 0003AB – Tuba City Headquarters quarters, 43 quarters, 40 gallon container required per quarter, twice a week (Monday and Thursday). Line Item 0003AC – Tuba City Boarding School (TCBS), building 296, 1 8 CY bin, twice a week . Line Item 0003AD – TCBS, building 289, 2 8 CY bins, twice a week. Line Item 0003AE – TCBS, building 309 (kitchen), 2 8 CY bins and 1 20 CY bins, twice a week. Line Item 0004 Kaibeto Boarding School. Line Item 0004AA – School building #400, 1 4X5X13 bin, twice a week (Monday and Thursday). Line Item 0004AB – Dormitory building #408, 1 4X5X10 bin, twice a week (Monday and Thursday). Line Item 0004AC – Kitchen building #413, 1 4X5X12 bin, twice a week (Monday and Thursday). Line Item 0004AD – Quarters, 4 quarters with 4 apartments in each, 19 quarter houses, 50 gallon bin for each, twice a week (Monday and Thursday). Line Item 0005 Rocky Ridge Boarding School. Line Item 0005AA – Kitchen, building #21, 2 22X7X8 bins, once every other week. Line Item 0006 Kayenta Community School Line Item 0006AA – Quarters (apartments), 8 units, 100 gallon bins, twice a week (Tuesday and Thursday). Line Item 0006AB – Quarters (houses), 37 units, 100 gallon bins, twice a week (Tuesday and Thursday). Line Item 0006AC – School building, Building 80, 1 6 CY bin, twice a week (Tuesday and Thursday). Line Item 0006AD – School building, Building 83, 1 6 CY bin, twice a week (Tuesday and Thursday). Line Item 0006AE – School building, Building 8152, 1 6 CY bin, twice a week (Tuesday and Thursday). Line Item 0006AF – Kitchen, building 148, 1 6 CY bin, twice a week (Tuesday and Thursday). Line Item 0006AG – Warehouse, building B-85, 1 8 CY and 1 40 CY bin, twice a week (Tuesday and Thursday). Evaluation factors include past performance and price, with past performance being less important than price. Provision 52.212-1, Instructions to Offerors-Commercial Items, applies to this acquisition. An addendum to this provision is applicable, in which the term offeror, whenever it appears, shall be replaced with the term quoter. Quoters are required to submit completed copies of the provisions at FAR 52.212-3, Offeror Representations and Certifications-Commercial Items, with their quotes along with a DUNS Number. FAR clause 52.212-4, Contract Terms and Conditions-Commercial Items, applies to this acquisition and does not include any addenda. FAR clause 52.212-5, Contract Terms and Conditions Required to Implement Statures or Executive Orders, Commercial Items, applies to this acquisition. The following clauses within 52.212-5(b) are applicable to this acquisition: 52.222-21, 52.222-26, 52.222-35, 52.222-36, 52.222-37, 52.232-33, 52.222-41. Service Contract Act Wage Determinations will be attached to the resulting purchase order. Required Central Contractor Registration and FAR clauses 52.219-6, Notice of Total Small Business Set-Aside and, Department of the Interior Acquisition Regulation (DIAR) 1452.226-70 Indian Preference, DIAR 1452.226-71 Indian Preference Program, DIAR 1452.204-70 Release of Claims are applicable. Before award can be made, the contractor will need to have registered in the Central Contractor Registration (CCR), located on the internet at www.ccr.gov. Quotes are due on February 22, 2005, 3:00 Mountain Daylight Time. Pricing will include all charges/fees to perform this requirement. Quotes will be submitted in writing and signed by an individual with the authority to bind their respective company.
