Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This solicitation is issued as a request for quotes (RFQ). Submit written offers (oral offers will not be accepted), on RFQ F2U3443450100. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2001-14 and Defense Acquisition Circular 91-13. This procurement is being issued as 100% small business set-aside. The North American Industry Classification System (NAICS) Code is 562998 and the size standard of $6Mil applies to this procurement. The requirement is to clean, inspect and repair three 500,000 gallon water storage tanks on Luke AFB, in accordance with the attached SOW. FOB Destination to Luke Air Force Base, Arizona 85309. The work will be completed within 60 days after the issuance of a notice to proceed. The provision at FAR 52.212-1, Instructions to Offerors--Commercial Items, applies to this acquisition. The evaluation will be based on low price, and technically acceptable offer. The clause at FAR 52.212-4, Contract Terms and Conditions-Commercial Items, applies to this acquisition. The clause at FAR 52.212-5 (Dev), Contract Terms and Conditions Required To Implement Statutes or Executive Orders--Commercial Items, applies to this solicitation. Specifically, the following clauses and provisions cited are applicable to this solicitation: 52.214-21, Descriptive Literature. Specifically, the following clauses, cited are applicable to this solicitation: DFARS 252.225- 7001, Buy American Act and Balance of Payment Program, DFARS 252.32-7003, Electronic Submission of Payments, DFARS 252.243-7001, Pricing of Contract Modifications. To be eligible to receive an award resulting from this solicitation, contractor must be registered in the DoD Central Contractor Registration (CCR) database, NO EXCEPTIONS. All quotes must be valid for 60 days and emailed to Jeffrey.kistler@luke.af.mil or faxed to (623) 856-3438 ATTN: TSgt Jeff Kistler. The response due date is 23 Feb 05. Quotes are required to be received no later than 4:00 PM PST, 23 Feb 05. STATEMENT OF WORK FOR THE INSPECTION AND CLEANING OF WATER STORAGE TANKS 1. SCOPE: The work covered by this statement of work consists of furnishing all plant, labor, equipment, appliances, and materials to inspect and clean 3 water storage tanks. The water storage tanks will remain operational during the inspection and cleaning. Perform all operations in accordance with OSHA and AWWA standards. All work will be in strict compliance the National Association of Corrosion Engineers (NACE), Steel Structure Painting Council (SSPC), the American Welding Society (AWS), and all terms and provisions of the contract. Air Force personnel will provide access to sites and be available during the inspection and cleaning procedures. The tanks are listed below: TANK SIZE HEIGHT TYPE 1 500,000 Gal 110’ Steel 2 500,000 Gal 220’ Steel 3 500,000 Gal On-Grade Steel 2. DISINFECTION: All personnel and equipment, which are placed in the water, will be completely disinfected with high concentration (200 ppm+) chlorine shower/scrub prior to entering the tank. 3. INSPECT: Interior inspection is to be completed by certified commercial divers specialized in water tank inspections and certified in ASNT Non-Destructive testing. Divers must use totally encapsulated vulcanized rubber dry suit with hardhat helmet isolating the personnel from the water at all times. The inspection will be completed with real-time closed circuit high-resolution color underwater video equipment. All pertinent findings will be recorded on VHS format videotape (including dive maintenance technician’s findings and narrative summary), as well as all checklists and written recommendations. Exterior tank inspection findings will be noted on the exterior reservoir checklist. 4. CLEAN: Completely clean accumulated bottom sediment from all tanks and dispose at contractor’s expense. Contractor will clean 1 inch of sediment/film from bottom and sides of tanks. The equipment must have rotating cleaning brushes incorporated in the vacuum head. The cleaning procedure and equipment shall not create any visible turbidity in the water. Contractor will submit price proposal for any additional amounts that need to be removed. 5. REPAIRS: Repair to coatings inside tanks will be made at the discretion of the contractor. All repairs, sealants and coating material must meet or exceed NSF 60 & 61 standards. 6. RECORDS: Videotapes, records, checklists and summary recommendations will be immediately provided upon completion of the operation and remains the property of Luke Air Force Base. PERFORMANCE PERIOD Work under this contract shall be completed within 60 calendar days after receipt by the contractor of written Notice to Proceed.
