THIS IS A SOURCES SOUGHT NOTICE. FOR FURTHER INFORMATION CONTRACT MR VINCENT PACCHIANA CLINICAL ANALYST AT 706-787-2094. This FEDERAL BUSINESS OPPORTUNITY announcement is a notice that TRICARE Southeast that include all DOD Medical Treatment Facilit ies (MTFs) in the states of Alabama, Florida, Georgia, Mississippi, South Carolina, Tennessee, and Southcom are in the process of standardizing Closed Ventilation Suction System as listed in the subject line above. Companies must obtain, or be in the proce ss of obtaining, a Distribution and Pricing Agreements (DAPAs) with the Defense Supply Center Philadelphia for these items to be eligible to participate in the standardization process. Technical Criteria: Please respond by Email: vincent.pacchiana@se.amedd.army.mil and provide your company name, address, point of contact, telephone, fax number, E-mail address and answers to the below listed questions by no later than March 14, 2005. 1. Does your company manufacture closed suction catheters? If so, what types of catheters does your company manufacture? 2. Are your products available through the Prime Vendor, Owens & Minor? 3. What is your DAPA number? 4. Under what brand(s) does your company manufacture or distribute your product? 5. Is your products latex free? 6. Are your products sterile? 7. Do your products have smooth edges? 8. Are instructions, part numbers, size and type readable on package? 9. Do your products have an accessible area for easy application of the change day/date label? 10. Do your products come in variety of sizes, single and double lumen, and configurations? 11. Separate needless lavage port. 12. Catheter fully retracts into the sheath 13. Suction control mechanism contains a non-stick, leak-proof and locking/protection feature. 14. Does your company provide customer service 24/7? 15. Is there any history of backorders and/or recalls for this product group?
