Description
STATEMENT OF WORK FOR CLINICAL SOCIAL WORKER 1. SCOPE: This contract is to provide for a treatment social worker for coverage in the Family Advocacy Program, Fort Detrick, Maryland. Provider is required to have a Masters Degree in Social Work or Doctorate in Clinical Psychology and possess a license in acco rdance with DODI 6025.13. Provider will become clinically privileged in accordance with MEDCOM directives, AR 40-68 and shall have a minimum of two years post-graduate clinical experience, including one of the last two years in family violence. Will offe r services to eligible beneficiaries of the Military Health Care System who present themselves to the FAP and/or the medical treatment facility. 2. Applicable Reference and Documents: a. All FAP SOPs and MOAs b. AR 608-18, Army Family Advocacy Program c. Accreditation Manual for Hopsitals JCAHO d. AR 40-68 QA Administration e. MEDDAC regulation 40-66, Quality Improvement Plan 3. Social Worker works under the guidance of the Family Advocacy Program Manager (FAPM). Clinical guidance will be provided by the Chief, FAP. The Social Worker will come to the position with knowledge and experience providing treatment services to fa milies who are affected by family violence. Duties are performed independently in accordance with accepted social work theories, models, techniques and practices. The social worker is expected to exercise initiative and sound judgement in establishing ev aluation techniques, developing treatment and rehabilitation plans and providing appropriate therapeutic treatment services. Social worker will keep FAPM informed on unusual cases or problems arising in connection with the rehabilitation of clients. 4. Major Duties a. Serve as a Clinical Treatment Social Worker on a highly specialized multi-disciplinary treatment team dealing with the FAP. Primary responsibilities include: (1) Providing psychosocial assessments (2) Conducting mental status examinations (3) Administration/Case Management for all treatment related aspects of the FAP (including monthly minutes, reports, records) (4) Goal-Setting (5) Intervention-Planning (6) Command consultation (7) Providing and Maintaining comprehensive psychosocial care of a highly specialized nature (FAP) as it relates to family violence (8) Participating in treatment program planning implementation and serving as the primary treatment provider (9) Providing policy formation and implementation (10) Participating in and conducting quality improvement measurements to insure consistent, high quality patient care. b. Utilize highly developed social work treatment skills for assigned patient populations to assess and resolve complex social, economic and emotional problems that may impact on FAP. Provide direct social work services, to include individual, marital, fa mily and group therapy. (1) Utilize knowledge and appropriate intervention skills to conduct psychosocial evaluations on each patient. Assess patients mental status, motivation, concerns, goals and impact of diagnosis on family relationships so as to identify areas needing reso lution for successful treatment to occur in compliance with the Case Review Committee (CRC) treatment plan. (2) Attend monthly case review committee meetings (CRC) and present applicable information on new or open cases. (3) Assume liaison responsibilities with other medical staff, civilian and military units and other community agencies to plan and coordinate patient care in compliance with the CRC treatment Plan. (4) Participate in development, implementation and documentation of quality improvement methods. c. Develop and modify programs within FAP to better provide assistance to the unique and complex needs of the assigned patient population. (1) Develop and/or refer patients to a victims group for victims of domestic violence. (2) Develop and/or refer patients to the mens batterers group for the perpetrators of domestic violence. d. Must attend/complete the Family Advocacy Staff-Training course and the Family Advocacy Staff Training Advanced course when selected for attendance. e. Other duties as assigned. f. Miscellaneous Information: (1) Other duties appropriate for performance by contract provider as directed or assigned by the contracting officer or his duly authorized representative. (2) The contractor will not exceed 40 hours per week. The specific hours to be worked will normally be Mon.  Fri., 0730-1630 with a one hour lunch break. However, the nature of the position requires flexibility in schedule hours and could include some evenings and weekends. Contractor is required to be available to take calls at home when there are family violence cases that occur during off-duty hours. The contractor is not required to work Federal Holidays. Any personal leave shall be coordinated w ith the FAPM. (3) Federal Law and Department of Army Circular 690-92-1, criminal history background checks on individuals providing care to children, 15 November 92, require that contract personnel involved with children undergo specific federal and state background c hecks. The contractor will be required to undergo State and Federal background clearances following the awarding of the contract. During the period when the background checks are in progress, the Family Advocacy Program Manager will implement specific se curity arrangements. Notes: Note 1: The contractor will not while on duty, advise, recommend or suggest to persons authorized to receive care at army expense while he/she is not on duty, or from a civilian associated in practice with the contractor. The contractor is not prohibited, by reason of his/her employment under this contract, from conducting a private practice, if there is no conflict with the performance of duties under the contract and government property in connection therewith. Note 2: Services rendered by the provider to eligible personnel will be at no charge to the patient. Note 3: The contractor hereby certifies that: (a) The contractor does not know of and has no reason to know of any pending suspension or debarment proceedings against it, any of its affiliates or any of its employees; (b) Neither the contractor nor any of its affiliates or any of its employees are now suspended or debarred from receiving government contracts. Note 4: The individuals providing service under the provision of this contract shall be approved by the commander, USAMEDDAC, via recommendations of the credentials committee utilizing standards as set forth by the Joint Commission for the Accreditation of Healthcare Organizations (JCAHO). Information about lack of professional conduct or competence shall be referred to the credentials committee. Note 5: The Healthcare Professional is solely responsible for the cost of his/her personal liability insurance. The healthcare professional also expressly releases and holds harmless the United States Government and all of its agents, including the Depa rtment of the Army, from all claims, demand, actions, judgements for personal injuries or property o whatever nature caused by or arising out of the services provided by the healthcare professional. Note 6: The contractor shall be responsible for filing quarterly Income Tax, Social Security Payments and for providing own health insurance. Note 7: The contractor is required to get a Common Access Card (CAC) which is used as an ID badge in order to gain access to the installation and computer equipment. The CAC requires a picture and fingerprints, which are kept in the Military Personnel Di vision.
