Description

The VA North Texas Health Care System, 4500 S. Lancaster Rd., Dallas, TX 75216 has a requirement for community hospice care for eligible veterans located in thirty-six (36) counties located in Texas and two (2) counties located in Oklahoma. Solicitation 549-15-04 will be isssued as a Request for Proposals in commerical item format. NAICS Code is 621610. Multiple contracts will be awarded from this solicitation. Period covered will be on or about January 1, 2005 through September 30, 2005 with four options to renew for a period of one year each. Solicitation will be available for download on or about 10/27/04 from the following website: www.fedbizopps.gov. Due date for proposals will be on or about 11/29/04. No hard copies will be available. Subject to the availability of funds. NOTE: THIS NOTICE MAY HAVE POSTED ON WWW.FEDBIZOPPS.GOV ON THE DATE INDICATED IN THE NOTICE ITSELF (19-OCT-2004).
