- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. This simplified acquisition procedures of FAR Part 13 will be used in conjunction with the commercial item procedures in FAR Part 12. The solicitation is being issued as a Request for Quotation (RFQ). The solicitation number is RFQ 246-05-01907. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 01-27. The NAICS is 517212. Items will be purchased and delivery will be made to the Department of Veterans Affairs Medical Center, 200 Veterans Ave., Beckley, WV 25801. Offeror?s proposals are hereby being requested in response to the requirements under firm-fixed price contract and shall be submitted on an offeror?s letterhead or bid form and shall contain the name, title, and signature of the person authorized to submit proposals on behalf of the contractor. Quotes are to be provided for 30 Cellular Telephones complete with both 110V and 12V automobile chargers and ear-type pods for hands-free communication. Furnish a local calling plan with at least 13,500 anytime minutes in the local calling area. Provide unlimited evening/weekend minutes in the local calling area and 800 roaming minutes to be used when outside of the local calling area. Free nationwide long distance unlimited mobile-to-mobile minutes. Free incoming text messaging with no more than $.40/minute overage rate or $.40/minute roaming rate over the 800 roaming minutes. The calling plan will be shared by all of the 30 telephones. Local coverage area will be the Beckley, WV geographical area extending into the surrounding counties of our ?catchment area? to include the counties of Mercer, Monroe, Greenbrier, Nicholas, Fayette, Clay, Summers, and Raleigh in West Virginia, and Giles and Tazwell counties of Virginia. Phones furnished will be 10 phones of a Motorola 60 Series flip-type phone or equivalent; 20 phones will be Nokia-3565 phones or equivalent. The non-local coverage area must include the major metropolitan areas of Beckley, WV; Salem, VA; Richmond, VA; Hampton, VA; Asheville, NC; Fayetteville, NC; Salisbury, NC; and Raleigh/Durham, NC. Also, major highways and throughfares between those areas. Quotes shall be valid for (30) days from the offeror?s due date. All responses must include the following: (1) descriptive literature and product number for the item offered providing the product offered meets the above requirements (2) warranty information (3) cost of product (4) shipping charges (5) delivery time after receipt of order. The following provisions and clauses apply to this solicitation: 52.212-1, Instructions to Offeror-Commercial Items, 52.212-4, Contract Terms and Conditions-Commercial Items, 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items, 52.233-2, Service of Protest, 852.233-70, Protest Content, 852.233-71, Alternative Protest Procedure. Award will be made to the offeror whose product meets the requirements listed above and is most advantageous to the Government. The Government intends to evaluate the information received and award without discussions. Offers including the pricing information are to be submitted on company letterhead and include the information required in provisions 52.212-3, Offeror Representations and Certifications-Commercial Items. In accordance with FAR 52.204-7 award will be made only to a contractor/company that is registered in the Central Contractor Registration (CCR). Electronic copies of the referenced provisions and clauses are available on the internet at www.fedbizopps.gov. Quotations and all required information must be received by 3:00pm local time February 28, 2005. Quotations shall be submitted in writing to Ruth B. Harris, Contracting Office (90C), VA Medical Center, 27 Averill Drive, Hampton, VA 23667. No fax or electronic offers will be accepted. Solicitation number RFQ 246-05-01907 shall be listed on the documents submitted for identification purposes. Only offers prepared in accordance with this solicitation shall be considered.
