Description
This is a Combined Synopsis/Solicitation, refer to solicitation number RFQ-618-117-05 entitled "Blood Culture System - 60 month Reagent Rental Agreement." This document is prepared in accordance with the format in Federal Acquisition Regulations (FAR) Subpart 12.6 Streamlined Procedures for Evaluation and Solicitation of Commercial Items, as supplemented with additional information included in this notice. This announcement incorporates provisions and clauses in effect through FAR FAC 2001-27 and VAAR 97-10. THIS CBD ANNOUNCEMENT CONSTITUTES THE ONLY SOLICITATION AND BID DOCUMENT THAT WILL BE ISSUED. A SEPARATE WRITTEN SOLICITATION BID DOCUMENT WILL NOT BE ISSUED. The VA will conduct this procurement in accordance with a combination of FAR Parts 12 and 13. The North American Industry Classification (NAICS) Code that applies is 325413 and the small business size standard is 500 employees. Contractors are requested to submit a written technical proposal and a cost proposal in response to the requirements listed below. A firm-fixed price contract is anticipated. The contractor shall submit his proposal with base year plus four 12-month options for a total contract period of 60 months. Proposal may be submitted on company letterhead or company bid proposal form. All offers submitted must contain the signature of a person authorized by the company to submit proposals. NOTE: The offeror MUST ADDRESS ALL ITEMS listed below in the VA's requirements, by submitting, in a separate technical proposal, an explanation of how the system and products that are proposed will meet each of the VA's minimum requirements. The Contractor's price and technical proposal must contain the name; title and signature of a person authorized to submit proposals/bids on behalf of the contractor. Descriptive literature for equipment offered must also be included in the proposal/bid package. If contractor offers these products on his Federal Supply Schedule, a copy of the FSS contract with the products highlighted shall be included in the proposal. Requirement: The VA Medical Center Minneapolis Minnesota Laboratory Service has a requirement of a "60-Month Reagent Rental Agreement" for a complete Microbiology Blood Culture System to include equipment and media bottles with broth. System must be capable of performing bacterial blood cultures with automated monitoring of cultures to detect growth and automated notification of lab staff when growth is detected. System must run on a 120V power system. Contractor's system will provide a safety inoculation device that is practical and easy to use. Systems must be capable of handling a minimum of 1100 bottles per month, 550 anaerobic and 550 aerobic. System must have a minimum capacity to hold 300 bottles at one time. The system provided and the culture bottles and media shall have a proven track record and a reputation of quality in the market. All culture media shall be of the highest quality and broth will contain a means of removing antibiotics effectively without interfering with gram stain or antibiotic sensitivity testing. Media culture bottles must have sufficient vacuum to allow for the blood sample to be added. The "Reagent Rental" agreement will provide on-site service and repair of system and software updates at no additional charge to the VA. Contractor will provide an on-site connection of the culture system to the VA Laboratory's LIS system. Training of end users is required, and on-site training of VA Laboratory staff is preferable. CONTRACTOR'S PROPOSAL MUST INCLUDE: (1) a Technical Proposal addressing each requirement listed above, to include service and maintenance that will be provided, as well as training, and connection to the VA's LIS system; (2) a Price Proposal with all stock numbers and a cost per culture bottle for the 60 month term; (3) Descriptive Literature, also include FSS contract if applicable; (4) three references including names and telephone numbers; (5) a completed copy of provisions 52.212-3 Offeror Representations and Certifications; (6) Provide the FOB point and shipping charges (if applicable); (7) Address time frames associated with, delivery time after receipt of an order, installation and training. (8) Prompt payment terms (9) Address ownership of equipment. Will contractor retain title of the equipment, or at the end of the 60 month term, will title be transferred to the VA Medical Center Minneapolis MN Evaluation of proposals will include the following evaluation factors: (a) The technical capability of products offered to meet the government's requirements as listed above (b) contractor's reputation in the trade and past performance history (c) price. Award will be made to the offeror determined to offer the "best value to the Government. Technical capability and past performance, when combined, will be weighted significantly higher than price. Evaluation may be made with or without discussions with the offerors. All responsible sources that can meet the requirements and provide the items as listed above may respond to this solicitation by submitting cost and technical proposals on their company letterhead or bid form. The following provisions and clauses shall apply to this solicitation: 52.252 Clauses Incorporated by Reference. This contract incorporates one or more clauses by reference, with the same force and effect as if they were given in full text. 52.212-1 Instructions to Offerors Commercial Items; 52.212-2 Evaluation - Commercial Items, 52.212-3 Offeror Representations and Certifications-Commercial Items (NOTE: OFFERORS MUST INCLUDE A COMPLETED COPY of "Offeror Representations and Certifications" along with offer or certify that his company has registered with ORCA.; 52.212-4 Contract Terms and Conditions - Commercial Items; 52.212-5 Contract Terms and Conditions required to Implement Statutes on Executive Orders - Commercial Items; VAAR 852.270-4 Commercial Advertising; 852.237-70 Contractor Responsibilities.
