Description

Full maintenance agreement for the following equipment: ADAC Laboratories Spect Total Body Imaging System S/N 9108285; Patient table S/N 1896130004; Sieko Color Video Printer S/N 27J77070; Codonics EP-1650 printer S/N 36C2025B; Hewlett Packard MOD Drive MD/Sure Store; IVY Biomed System MD/101T S/N 991226046; ADAC Ultra Workstation MD/2153-3000A S/N 99091303; ADAC Ultra Workstation MD/N-P-Ultra-HT S/N P02031549. Contract Service shall ensure that the equipment functions in conformance with conformance with the latest published edition of NFPA-99, OSHA, and the original equipment manufacturer_s performance standards and specifications. The contractor will provide repair service which will consist of calibration, cleaning, lubricating, adjusting, replacing parts and maintaining the equipment including all intervening calls necessary between regular services and calibrations. All required parts will be furnished. Period of performance shall be from date of award until 30 Sep 05. Four option years will be included which will begin on 1 October and end on 30 September. Note 1 applies. NAICS Code is 811219.
