Description
This is a combined synopsis/solicitation for commercial requirements prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the ONLY solicitation; quotes are being requested and a written solicitation will NOT be issued. The Department of Veterans Affairs (VA), Southern AZ VA Health Care System (SAVAHCS) Surgical Section, 3601 S. 6th Avenue Tucson, Arizona is seeking quotes from qualified U.S. Small Businesses to provide quality Spinal Surgery Table equipment. The SAVAHCS needs a Jackson Table System (JTS) or equivalent, which is an advanced spinal surgery positioning modular table system. The table system shall have a radiolucent frame with support pads; it shall allow anterior, posterior, lateral, and oblique views intra-operatively; and shall permit other fluoroscopic procedures. The table system must include customized support cushions to enhance patient positioning and must facilitate safe and efficient (360 degrees) rotation of the patient. The SAVAHCS requires the following accessories: 1 Each, Advanced Control Retractable ?I? Base (5803); 1 Each, Radiolucent Imaging Top with Gimbals, 500 lbs. Patient load (5927); 1 Each, Jackson Spinal Surgery Top Gimbals with ShearGuard, 500 lbs. Patient load (5943); 2 Each, Pivoting arm Board 3? with ShearGuard Gel Pad (5357); 1 Each A/P Cassette Tray (5839); 1 Each, Patient Care kit, case of 6 (5808); 2 Each, Side Rail adapters (5855-830); 1 Each, Cervical Management Base Unit (5879-1); 2 Each, Retractor adaptors (5888); 1 Each, Spetzler Skull Clamp with pins (5893); 1 Each, Gel Table Pad, Hip Section 20in. x 20in. (10101); 1 Each, Anterior Extension Pillow with 5 inch bladder (5879-2); 1 Each, Supine Top, short (5880-09); and an Extended Warranty for the JTS. The Contractor shall provide quotes for the required table system and quotes for the required accessories and extended warranty. The applicable NAICS for this requirement is 339113. The Government will award a Firm-fixed price Contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government. The Contractor shall provide a complete copy of the Offeror Representations and Certifications?Commercial Items (FAR 52.212-3 JAN 2005) with its offer. Many of the provisions and clauses cited in this solicitation are incorporated by reference, FAR 52.252-2 (FEB 1998), to the appropriate section of the FAR and VAAR.
