Description
SYNOPSITATION FOR PREVENTIVE AND REMEDIAL MAINTENANCE OF LABORATORY EQUIPMENT, FA8601-05-R-0032 This is a combined synopsis/solicitation for the preventive and remedial maintenance and repair of laboratory equipment at the Air Force Institute of Technology (AFIT), Wright-Patterson Air Force Base, prepared in accordance with (IAW) the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This procurement will be awarded in accordance with FAR Subpart 13.5, Test Program for Certain Commercial Items. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation FA8601-05- R-0032 is a Request for Proposal (RFP). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2001-26. Proposed acquisition is unrestricted, full and open competition. The associated North American Industrial Code (NAIC) is 811219/Standard Industrial Classification (SIC) code 3829, size standard $6 Million applies. Due date for receipt of proposals is 11 Mar 05, 3:00 pm EST. A Firm Fixed Price type contract is anticipated with five contract line items (CLINs) as follows. The anticipated contract will have one base year and four one-year options. CLIN 0001 for the period 12 Apr 05 ? 11 Apr 06. As described in the Statement of Work contractor will provide all personnel, equipment, tools, parts, and supervision necessary to maintain and repair equipment listed in Appendix A to the Statement of Work. Some relocation of equipment may also be required. CLIN 0002 for the period 12 Apr 06 ? 11 Apr 07. Description same as CLIN 0001. CLIN 0003 for the period 12 Apr 07 ? 11 Apr 08. Description same as CLIN 0001. CLIN 0004 for the period 12 Apr 08 ? 11 Apr 09. Description same as CLIN 0001. CLIN 0005 for the period 12 Apr 09 ? 11 Apr 10. Description same as CLIN 0001. NOTE: Pricing for repair parts should not be included in proposal. These costs will be billed separately at invoice cost as described in paragraph 1.2 of the Statement of Work. Pricing for an estimated 12 transducer calibrations to be performed on 4 separate service calls should be included in proposal on all CLINs. Commercial. ADDENDUM TO 52.212-1: The following shall constitute a complete proposal package. If any one portion of a proposal package is missing, the proposal may be considered nonresponsive and not considered for award: a. Price Proposal b. A description of preventive maintenance and repair activities included in your pricing. This description must be sufficient to allow Government evaluators to determine if your proposal is technically acceptable and meets requirements of the Statement of Work. c. At least three (3) completed Past Performance Questionnaires. The questionnaire is attached at www.pixs.wpafb.af.mil. Offerors must send this questionnaire to at least 3 past/present customers and instruct them to complete the questionnaire and fax it to Helen Williams at 937-656-1412. Offerors are responsible for ensuring that at least three completed Past Performance Questionnaires are received by Helen Williams by the closing date/time for receipt of proposals, 11 Mar 05, 3:00 pm EST. d.
