Description
SOLICITATION NOTICE for Ft. Wainwright, Alaska 99703: This is a combined synopsis/solicitation in accordance with FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicit ation; quotes are being requested and a written solicitation will not be issued. This solicitation will be issued as a request for quotation (RFQ) under the following solicitation number: W91CZ-05-T-0024. This solicitation and the provisions and clauses incorporated are those in effect though the Federal Acquisition Circular (FAC) 2001-26. This acquisition is a 100% Small Business Set Aside for - SIC code: 2299; NAICS CODE: 339999 Size Standard: 500 EMPLOYEES. Closing date is 15 MARCH 2005 at 12:00 PM (Alaska Time). This RFQ is for the purchase of seven hundred and seventy (770) combat lifesaver bags with the following required characteristics: Compartmentalized system that can be worn on the chest, attached as a butt pack, or be worn over the abdom inal area. Must be compatible with the Modular Lightweight Load Carrying Equip (MOLLE) system. The bag must be capable of being fully secured to the combat lifesaver (CLS). CLSs must have the ability to open and use the bag when it is worn on their pers on. It must contain compartments that will secure the Medical Equipment Set (MES) CLS components after the bag is open and while the combat lifesaver is moving in between patients. The MES CLS consists of (listed for vendor knowledge, quote shall not inc lude these components): 5 Each Atropine Inj 0.7 ML, 2 Each Hetastarch IN Lactate, 5 Each Diazepam Inj 2 ML 10S, 2 Each Dressing First Aid, 4 Each Bandage 37x37x52IN, Dress Fld 11-3/4, 18 Each Bandage Adhesive 3, 12 Each Pad Isopropyl Alcohol, 2 Each Adh esive Tape Surg 1, Pad POV-IOD Impre100S, 12 Each Bandage ELAS 3 Wide, 6 Each Sponge Surg 2x2 100S, 1 Each Airway Phray 100MM10S, 1 Each Scissors Bandage 7.25, 1 Each Airway Pharyn 80MM10S, 1 Each Tube Drain 1x18 200S, 4 Each Catheter/NDL 18GA 50S, 2 E ach Intravenous Injection, 10 Each Patient Exam Glove 100, 1 Each Universal Splint, 1 Each Case#3 10x4-1//2x18IN. It must have a hook and pile or like attachment for complete and rapid opening and closure in case of emergency movement. It must have a g rommet style arrangement to allow for drainage of sand and water at the base of the bag. The bag must have sufficient compartment space to allow for the functional division of MES CLS components. It must be large enough to allow for the addition of 25% e xcess of MES CLS components (see above for the component listing). The empty CLS bag should not weigh more than five pounds nor the fully-loaded bag impede the movement of the combat lifesaver while walking or running. Shipping/Delivery Charges should be quoted for delivery to Fort Wainwright, AK 99703 if applicable. Delivery Date: By 30 March 2005 or sooner.
