- NA SOURCES SOUGHT/SOLE SOURCE Initially, this is a combined synopsis/solicitation for ?sources sought? wherein the Department of Veterans Affairs Veterans Integrated Service Network 23 (VISN 23), Minneapolis, MN is seeking vendors who can provide collaborative leadership, teaching and coaching for Medical Teams in VISN 23. THIS WILL BE A 12 MONTH CONTRACT WITH AN OPTION FOR 6 MONTHS. THE OPTION YEAR IS SUBJECT TO THE AVAILABILITY OF FUNDS FOR FISCAL YEAR 2006 SCOPE OF WORK: Contractor Responsibilities: The envisioned collaborative will center around improving nurse-physician communication in the teams involved, improve either timeliness or efficiency of a selected process, to improve nurse-physician leadership and finally to spread these improvements to staff ?back home?. The Contractor will provide an expert, or experts, in the specific area of communication between members of Clinical Teams consisting of physicians, nurses and allied health professionals. The Contractor?s representative (i.e., Consultant) will have substantial demonstrated and documented experience and success in planning, conducting and coordinating critical care quality improvement collaborative. The Consultant must have substantial demonstrated and documented experience with the ?Plan-Do-Study-Act? (PDSA) model of improvement. The VISN 23 Clinical Team Communication Collaborative is designed based on the model of the Institute for Health Care Improvement?s ?Breakthrough Series? quality improvement collaborative. Therefore the Consultant must have demonstrated and documented familiarity and experience using this model. The Consultant will provide on-going guidance on the overall continuing management of the Collaborative. General: Read and review all existing Collaborative documents (Collaborative Charter, Change Package, Measurement Package, Resource Manual and Pre-work Packet). The Consultant will participate in the planning process, provide information on Collaborative subject matter and its application, as well as methods for process improvement, both during and between Learning Sessions, offer coaching to teams, assess team?s progress on a monthly basis and provide timely feedback to the Collaborative Planning Committee. The Consultant will identify problems and barriers to improvement and make additional recommendations to improve care (for example through the use of existing or proposed information technology systems including clinical information systems). The Consultant will shadow and provide coaching to the VISN Collaborative leadership/planning team via email, Internet, phone and on-site as necessary. The Consultant will provide coaching and guidance during Collaborative calls and review teams? progress monthly along with designing appropriate interventions. The Consultant will review and make recommendations on the Collaborative website and data collection methods. The Consultant will participate in at least once monthly 60-minute planning calls with the Collaborative Planning Committee and at least once monthly 60-minute all-collaborative calls intended to provide coaching to teams. The Consultant will attend in person Learning Session I on May 18 & 19, 2005 in Sioux Falls, SD and two additional learning sessions (Learning Sessions II and III) over the duration of the contract, also in Sioux Falls, SD. During Learning Sessions: Learning Session II & III: Consultant will actively assist in developing timelines, objectives, agendas, and evaluations. Learning Sessions I, II & III: Consultant will review and provide plenary and small group presentations, and written materials as appropriate along with feedback on the modification of Collaborative plans and materials for both the front-line teams and the leadership (COS-CNE) group. The Consultant will provide samples and templates of presentations, review and provide feedback on presentations and will advise the VISN leadership team on applying the PDSA model for rapid cycle improvement to test, spread and implement changes at the local level. Learning Session I, II & III: The Consultant will provide guidance to the Collaborative Planning team in planning and facilitating an effective breakout session designed for senior level VA facility and inpatient clinical teams. The intention of this presentation and breakout session will be to further develop communication skills among facility level leadership and to facilitate development of their team?s improvement efforts and applaud their accomplishments. This session will focus on the key role of leadership in collaborating with and mentoring front-line staff to ensure the spread of positive change and to improve patient safety, quality of patient care, and efficiency throughout their facilities through improved clinical team communication. An additional aim will be to provide education regarding leadership?s role in supporting multidisciplinary teams of front line employees in creating positive change through the use of rapid-cycle testing. During Action Periods: The Consultant will provide recommendations for designing and implementing action period activities, will review VISN plans, and provide feedback. Action Period Activities may include e-mail communications, conference calls, and reporting, which are designed to provide support and momentum for teams as they adapt and test ideas for improving the system of care. A particular focus of the Consultant will be to provide coaching, education, and guidance to the Collaborative Planning/Leadership group on tools and methods of effective coaching in guiding individual teams through the improvement process, facilitating conference calls and meetings and in helping individual teams when necessary. THE CONTRACTOR IS RESPONSIBLE FOR CONSULTANT TRAVEL COSTS. Collaborative Evaluation The Consultant will review the Collaborative Planning team?s plan for evaluating the overall success or failure of the Collaborative and will provide feedback and guidance for modifying the plan. Assist with development of presentation to VISN Leadership demonstrating impact of the Collaborative in improving patient level or organizational targets. The Consultant will provide samples and templates for developing a system to measure the teams' overall performance during the Collaborative. Reporting: The Consultant will participate in a brief meeting at the end of each Collaborative call to review and critique the call and to offer suggestions for improvement of future calls. A written follow up report will be sent via e-mail within 5 working days of the call. The Consultant will review the final agenda for each Learning Session and will send comments/suggestions in writing via e-mail within 5 working days of receipt. Within 10 working days after each Learning Session the Consultant will provide a written report to include a critique of the Learning Session and suggested methods for improvement. The Clinical Team Communication consultant will provide written interim reports to VISN leadership at 6 a final report at 12 months to include evaluation and recommendations and will assist in development of final presentation to VISN leadership. Any large and all types of small business vendors may respond to this ?sources sought? announcement. Provide a detailed written and signed proposal on company letterhead or other proposal forms, including pricing, for these items. You are further required to provide Representations and Certifications as identified below. Failure to provide ?Reps and Certs? may make your offer non-responsive and delay award of contract. Refer to FAR 52.215-1(f)(4) regarding the actions permitted the Contracting Officer to consider the number of proposals exceeding the number at which efficient competition can be conducted. EVALUATION FACTORS APPLICABLE TO THIS SOLICITATION (all Offerors must respond): (1) Technical capability of the services offered to meet the Government requirement; (a) Offeror will provide credentials for expert(s) provided under this contract as defined above in Scope of Work. (b) Offeror must provide documented experience of the contractor?s representative as defined above in Scope of Work. (c) Offeror to provide documented experience as an expert in: ?ABCs of Effective Communication?; ?HIBWs?; and ?Caring Matters? (2) Past performance: Identify Federal, State, local government contracts and/or private contracts of similar scope, size, and complexity that are ongoing, or have been completed within the past 3 years. Provide a list of three references pertaining to those contracts including name, address, contact person, and telephone number (including area code). Include information that depicts contract compliance, financial stability and efficiency, and customer satisfaction. Offerors with no relevant performance history will receive a neutral rating. (3) Small disadvantaged business participation. Credit under this factor is not available to an SDB concern unless it specifically waives the price evaluation adjustment as indicated in FAR clause 52.219-23.and; (4) Price. Technical, past performance and small disadvantaged business participation factors, when combined, are approximately equal to price. IN THE EVENT THAT NO ADDITIONAL SOURCES ARE IDENTIFIED VIA THE ?SOURCES SOUGHT? PORTION OF THIS ANNOUNCEMENT by March 18, 2005 the Government anticipates entering into sole source procurement negotiations with Tememos, Inc. Honolulu, HI to provide collaborative coaching for Clinical Team Communication facility teams in VISN 23. THIS WILL BE A 12 MONTH CONTRACT WITH AN OPTION FOR 6 MONTHS. THE OPTIONAL CONTRACT IS SUBJECT TO THE AVAILABILITY OF FUNDS FOR FISCAL YEAR 2006. SCOPE OF WORK above. Therefore, due to lack of additional sources, this becomes a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6 as supplemented with additional information included in this notice under Statutory Authority 41 U.S.C. 253(c)(1) and in accordance with FAR 6.302 and VAAR 810.005. The authority is cited and justification made based on the determination that the Government?s minimum needs can best be satisfied by Sole Source Only services. This procurement is being conducted under FAR Subpart 13.5 Test Program; simplified acquisition procedures apply. VA will conduct the procurement according to a combination of FAR Part 12, Acquisition of Commercial Items and Part 13, Simplified Acquisition Procedures as supplemented with additional information included in this notice. THIS ANNOUNCEMENT CONSTITUTES THE ONLY SOLICITATION; A WRITTEN SOLICITATION WILL NOT BE ISSUED BUT REFER TO SOLICITATION # RFQ 618-120-05. Firms may identify their interest and capability to respond to this requirement by 4:30 p.m., March 18, 2005. This notice of intent is a request for Sole Source Only competitive proposals.
