- 4890 University Square. Suite 3f, Huntsville, AL 35816 Description This is an amendment to the original combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, in conjunction with FAR 13.5, and as supplemented with additional information included in this notice. This synopsis/solicitation is now for the Mid IR2 Laser only. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The solicitation number is 4THO57053058 and is issued as a request for quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular FAC 01-24. The associated North American Industrial Classification System (NAICS) code for this procurement is 541690 and 334413, with a small business size standard of 1,000 employees. However this requirement is unrestricted and all interested offerors may submit a quote. Quotes shall be submitted no later than February 21, 2005. The GSA FTS Region 4 requires the following items Item 1) Coherent, Inc. MID-IR2 Laser Part # 1040081 Qty: 1 Past performance will be considered and evaluated as part of this acquisition, per FAR 12.206. As part of your quotation package, you must submit two (2) examples of task/delivery orders or contracts you have performed within the last two years or are currently performing, which are similar to the requirements of this RFQ. Make sure to include a point of contact name for the client organization, as well as telephone number and e-mail addresses. A past performance questionnaire will be sent to these POCs for performance verification. Basis for award will be made on price and other factors, to include past performance. Quotation evaluation will be based on price and past performance, and award will be made based on whether the lowest price of the quotations having the highest past performance rating possible represents the best value when compared to any lower priced quotation. In addition to providing pricing for this solicitation, each Offeror must provide any required NON-PRICING responses (e.g. technical quote, representations and certifications, etc.) directly to kristen.eaves@gsa.gov so that they are received at that email address no later than the closing date and time for this solicitation. The selected Offeror must comply with the following commercial item terms and conditions. FAR 52.212-1, Instructions to Offerors - Commercial, applies to this acquisition. The offeror must complete the annual representations and certifications electronically via the ORCA website at http://orca.bpn.gov. After reviewing the ORCA database information, the offeror verifies by submission of this offer that the representation and certifications currently posted electronically at FAR 52.212-3, have been entered or updated in the last 12 months, are current, accurate, complete, and applicable to this solicitation (including the business size standard applicable to the NAICS code referenced for this solicitation), as of the date of this offer and are incorporated in this offer by reference. The following FAR clauses in paragraph (b) of FAR clause 52.212-5, Contract Terms and Conditions Required To Implement Statutes or Executive Orders-Commercial Items, will apply: 52.222-21, 52.222-26, 52.222-35, 52.222-36, 52.222-37, 52.225-13, 52.232-34. CCR Requirement - Company must be registered on Central Contractor Registration (CCR) before an award could be made to them. If company is not registered in CCR, they may do so by going to CCR web site at http://www.ccr.gov. Buyers and Sellers understand that a Buyer always maintains all rights assigned under their contracting authority and/or applicable acquisition regulations, and may select the most advantageous bid for the government’s purpose while considering offline factors such as delivery, past performance, socio-economic classifications, and source rotation. Vendors / sales agents submitting quotes MUST BE registered in ITSS. If vendor registration assistance is needed contact 877.243.2889 Option 2 Quote per specifications requested. If an item has been discontinued or is end of life, annotate quote referencing as such then quote an equal or better replacement for the item. Quote is to include all fees, including shipping or freight costs. Provide on quote the estimated ETA (number of days) when delivery can be expected after receipt of award document.
