Description

The VA Medical Center?s Vet Center in Syracuse, NY has the need for a contractor to provide Readjustment Counseling Services in the following counties in the state of NY: (note: numbers in ?( )? signify the estimated quanity of patients receiving services under this contract) St. Lawrence (50), Jefferson (35), Lewis (5 or less), Herkimer (5 or less), Oneida (5 or less), Oswego (5 or less), Onondaga (5 or less), Madison (5 or less), Cayuga (5 or less), Wayne (5 or less), Tompkins (25), Cortland (5 or less), Chenango (5 or less), Broome (5 or less) and Tioga (5 or less). The contractor will be required to furnish approved therapy programs for the set fees. Service providers submitting offers in response to the request for proposal must be physically located where a Vet Center is not present or in counties that are being serviced by a Vet Center and must be an established, licensed business, with a proven track record of providing such services in the counties listed above. Contract is for the period of April 1, 2005 through Mach 31, 2006 (or upon award of contract) with two (2) one year options, pending availability of funds. The NAICS code is 624190. The small business size standard is $5 Million. Solicitation will be issued on or about March 4, 2005. Response due March 21, 2005 @ 4:00 PM EST.
