Description
The Centers for Medicare and Medicaid Services (CMS) intends to award on a sole source basis, under the Simplified Acquisition Procedures, (Dollar Threshold of $100,000) a firm fixed price award to Dr. Judith H. Hibbard, University of Oregon, Department of Planning, Public Policy & Management, Eugene, Oregon 97403-1209. The purpose of this statement of work is to provide an early assessment of beneficiary knowledge and awareness of the Medicare Prescription Drug Benefit (MPDB) and to identify characteristics of beneficiaries who may either have difficulty understanding or securing assistance in making their choices or who may be disinclined to enroll in a Medicare Prescription Drug Plan (MPDP). This project will build on a related AARP study. The AARP funded study involves the development and testing of Medicare knowledge items which will be used in a survey of beneficiaries fielded in the fall of 2005. This study will test some of these same items developed for the larger fall survey Hibbard and colleagues (2005) have developed a screening index to assess health literacy in older adults. The three variables in the weighted index are: respondent age, self-rated health status, and education. The index can be used to quickly and easily identify those who have marginal or inadequate literacy. It has a relatively high level of both sensitivity and specificity for predicting health literacy levels. In a recent study seventy-one percent of respondents are correctly classified by the screening index as either a true positive (low health literacy) or a true negative (adequate health literacy). The Patient Activation Measure (PAM) is a 13-item measure, developed by Judy Hibbard that assesses patient knowledge, skill and confidence for self-management. The measure was developed using Rasch analyses and is an interval level, uni-dimensional, Guttman-like measure. The measure has strong psychometric properties and is predictive of subsequent health behaviors and disease-specific self-management behaviors. There are several reasons why an open competition is not feasible for this contract. First, the study builds on a current AARP study under the direction of Dr. Judith Hibbard. Second the contracted work calls for the use of the Patient Activation Measure (PAM). This measure was developed by Dr. Hibbard and colleagues at the University of Oregon. Their team has more experience than any other in using the measure and interpreting findings. Finally, the work also calls for the use of a new instrument that is used to screen for health literacy in an older population. This tool was also developed by Hibbard and her team. Given the nature of the proposed work, it would be impossible to find another team that would match expertise and experience found on the Hibbard team.
