- This is a combined synopsis/solicitation for commercial items prepared in accordance with the information in FAR 12.6, using Simplified Acquisition Procedures as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; a written solicitation will not be issued. The RFQ number is number N00259-05-T-0025. This solicitation document and incorporated provisions and clauses are those in effect through FAC 2005-01 and DFARS Change Notice 20050222. It is the contractor?s responsibility to be familiar with the applicable clauses and provisions. This is a HubZone Set-Aside. Naval Medical Center San Diego (NMCSD) requests responses from qualified sources capable of providing: CLIN: 0001BASE YEAR: NON-PERSONAL SERVICE for an Ophthalmology/PRK SURGERY COORDINATOR in accordance with the statement of work(SOW). QUANTITY: 1,016, UNITof ISSUE: HOUR, UNIT PRICE: ________, EXTENDED AMOUNT:________.The period of performance is from 04 April 2005 ? 30 September 2005. CLIN: 0002 OPTION YEAR ONE: NON-PERSONAL SERVICE for an Ophthalmology/PRK SURGERY COORDINATOR in accordance with the statement of work(SOW). QUANTITY: 2,008, UNITof ISSUE: HOUR, UNIT PRICE: ________, EXTENDED AMOUNT:________.The period of performance is from 01 October 2005 ? 30 September 2006. STATEMENT OF WORK ?SURGERY COORDINATOR SECTION C-1 CONTRACT SPECIFICATIONS 1.1 Scope of Work. The Government is seeking to place under contract, an individual or business entity that can provide scheduler, clerical, and administrative services in support of the Department Head in Ophthalmology, Naval Medical Center San Diego (NMCSD), 34800 Bob Wilson Drive, San Diego, CA, 92134. 1.2 Independent Contractor. This is a non-personal services contract as defined in FAR 37.101, the services rendered by the contractor are rendered in the capacity of an independent contractor. The Government will evaluate the quality of both professional and administrative services for purposes of contract inspection and acceptance. The Government retains no control over the professional aspects of services rendered (e.g. professional judgments). The contractor shall be solely responsible for any and all liability caused by the acts or omissions of its agents or employees. The contractor shall not in any manner represent or infer that it is an instrumentality or agent of the United States Government. The contractor shall recognize the Commander, Naval Medical Center San Diego maintains administrative and operational responsibility for all activities within the command and may take such actions as necessary to preserve and maintain the integrity of the command, subject to the limitations prescribed by law and U.S. Navy Regulations. 1.3 Brief Description of Services. The contract employee is a scheduler to the Department Head in Ophthalmology, and performs a of clerical and administrative duties, such as filing, inputting data, and scheduling which are auxiliary to the work of the organization. The work unit is small to medium sized. The scheduler will have extensive responsibility for coordinating work outside the organization for coordinating work inside the organization, where procedures and administrative controls are equivalent to those typical of a larger, more complicated office. 1.4 Quality of Service. The services specified in this contract shall be performed in accordance with established principles, practices and ethics of the departmental surgery coordinator profession and any written policies, procedures and requirements of the Naval Medical Center San Diego. 1.5 Inspection and Acceptance. Performance will be reviewed by the GPOC for adequacy, completeness, and accuracy, and whether it meets the objectives set forth in this statement of work. 1.5 Place of Performance. Work will be performed on-site at NMCSD?s Ophthalmology Department. 1.6 Period of Performance. The period of performance will be six (6) months; 04 April 2005 through 30 September 2005. 1.7 Hours. Contractor will submit monthly invoice for hours expended per CLIN 0001. SECTION C-3 GOVERNMENT FURNISHED PROPERTY AND SERVICES. The government will provide, without cost to the contractor, the facility, equipment and/or services listed below which the contractor shall use for the performance of this contract. 3.1 Facility. Building 2-2, room number Ophthalmology Department. 3.2 Equipment. Dell computer s/n CB55H41 and monitor, standard phone, and Epson regular Fax machine. 3.3 Services. The Government will provide station telephone services for official use only, and intra command distribution of mail and will provide outging mail service for offical Government correspondence. 3.4 Orientation. The Government will provide an eight-hour Command orientation for contract employees assigned to this contract. The orientation will take place within six months of contract award. Contract employees are required to attend this 8-hour program. If the successful contractor has previously attended this orientation, the requirement will be waived. SECTION C-4 CONTRACTOR FURNISHED ITEMS. 4.1 Insurance. Prior to start of performance, the contractor shall provide to the contracting officer proof that the required insurance has been obtained. Reference is made to clause 52.228-5 ?Insurance-Work on a Government Installation?. The insurance requirements specified will be applicable to the contractor and any subcontractors. The contractor shall comply with the Workers Compensation Insurance Laws of the State of California. SECTION C-5 SPECIFIC TASKS 5.0 General Principles: 5.1 Coordinates the surgical schedule for the Ophthalmology department for all surgery patients and submit schedules in a timely manner to the operating rooms for sequencing of patients. Must also schedule the preoperative appointments, surgery date, and post operative appointments as well as ensuring patient has completed all required paperwork. 5.2 Notifies patients of appointment date, time, location, and any other pertinent pre-appointment/surgery instructions 5.3 Assists patients with completing required forms, also providing all necessary information. 5.4 Tracks staff Temporary Additional Duty (TAD) and Leave requests, and coordinates surgery/clinic schedules accordingly. 5.5 Uses either a word processor or a typewriter, types narrative and tabular material, establishes and maintains computerized surgery databases. 5.6 Maintains subject matter files and records that relate to the work of the unit. Sets up and maintains logs showing the status of actions and correspondence. Ensures files include all required documents and that all documents are properly signed. 5.7 Locates and pulls records for providers upon request. 5.8 Receives visitors and telephone calls to the office, determines the nature of the requests and directs callers to appropriate staff, or personally providing the information desired when routine or procedural matters of the office are involved. 5.9 Responds to routine and non-technical requests for information such as status of reports, suspense dates for matters requiring compliance, and similar information readily available from files. This may involve determining whether or not to release the information based upon the nature or degree of sensitivity of the information and the requester's authority to receive the information. 5.10 Prepares Same Day Surgery packets. 5.11 Distributes and collects department mail. 5.12 Attends all mandatory command and departmental meetings. 5.13 Provide administrative support duties to the Refractive Surgery Clinic (PRK clinic) Typical are the following: 5.13.1 Communicates regularly with the PRK clinic schedulers Division Officer. Acts as liaison for Refractive Surgery Center and Ophthalmology surgery schedules. Maintains surgical calendar of Ophthalmology surgeries and Refractive surgeries/consent/PRK clinic days and provides weekly and also monthly schedule to surgeons/PRK clinic schedulers, as well as notify immediately if any changes. 5.13.2 Prepares Refractive Surgery consent areas at Ophthalmology Department and ensures that paperwork is completed by patient and signed by surgeon and copies are made. 5.13.3 Contacts Refractive Surgery patients one day prior to their surgery to confirm surgery arrival times/location. SECTION C-6 PERSONNEL QUALIFICATIONS 6.0 Contractor Qualifications 6.1 Knowledge of the organizational functions and procedures for the work unit to perform such duties as distributes and control mail, refer phone calls and visitors, and provide information. 6.2 Knowledge of patient contact principles/techniques, patient record organization common courtesy, telephone etiquette, military protocol, and working cooperatively. 6.3 Knowledge of appropriate medical technology and ophthalmic procedures. 6.4 Customer oriented. Must have good verbal/written communication skills of the English language. Must be able to communicate effectively also with patients hard of hearing. 6.6 Knowledge of the appointment and log system in the department and knowledge of the CHCS computer operation 6.7 Knowledge of the Excel, Access, Word, and Outlook computer programs. 6.8 Knowledge of the Computer, Fax machine, photocopier, shredder, phone systems. 6.9 Knowledge of correspondence requirements to check for proper grammar, spelling and punctuation, and to review documents for conformance with formats and procedural requirements. 6.10 Knowledge of the office procedures, filing system and handbooks commonly used by the staff in order to classify, search for, and dispose of materials. SECTION C-7 OTHER PROVISIONS 7.0 Administrative Requirements 7.1 Hours of operation. The Ophthalmology department?s operating hours are 0730-1600, Monday through Friday. 7.1.1 Contractor services will not be required on the following federally established holidays: New Year?s Day, Martin Luther King?s Birthday, President?s Day, Memorial Day, Independence Day, Labor Day, Columbus Day, Veteran?s Day, Thanksgiving Day and Christmas Day. The government will not compensate the contractor for these periods of planned absences. 7.2 The contractor shall become acquainted with and obey all station regulations, shall perform in a manner to preclude the waste of utilities, and shall not use Government telephones for personal business. 7.3 All motor vehicles operated on these installations by the contract workers shall be registered with the base security service according to applicable directives. Eating is prohibited in patient care areas/clinics and is restricted to designated areas. Smoking is prohibited in the facility and is restricted to designated outdoor areas. 7.4 The contract workers shall display an identification badge, which includes the contract worker?s full name and professional status (furnished by the Government) on the right breast of the outer clothing. Security badges provided by the Government shall be worn when on duty. 7.5 The Secretary of the Navy has determined that the illegal possession or use of drugs and paraphernalia in a military setting contributes directly to military drug abuse and undermines Command efforts to eliminate drug abuse among military personnel. The policy of the Department of the Navy (including the Marine Corps) is to deter and detect drug offenses on military installations. Measures to be taken to identify drug offenses on military installations, and to prevent introduction of illegal drugs and paraphernalia, include routine random inspection of vehicles while entering or leaving, with drug detection dogs when available, and random inspection of personal possessions on entry or exit. If there is probable cause to believe that the contract worker has been engaged in use, possession, or trafficking of drugs, the contract worker may be detained for a limited period of time until he/she can be removed from the installation or turned over to local law enforcement personnel having jurisdiction. When illegal drugs are discovered in the course of an inspection or search of a vehicle operated by the contract worker, the contract worker and vehicle may be detained for a reasonable period of time necessary to surrender the individual and vehicle to appropriate civil law enforcement personnel. Action may be taken to suspend, revoke, or deny installation driving privileges. Implicit with the acceptance of this contract is the agreement by the contract worker to comply with all Federal and State laws as well as regulations issued by the Commander of the military installation concerning illegal drugs and paraphernalia. 7.6 All financial, statistical, personnel, and technical data which are furnished, produced or otherwise available to the Contractor during the performance of this contract are considered confidential business information and shall not be used for purposes other than performance of work under this contract. Such data shall not be released by the Contractor without prior written consent of the COR. Any presentation of any statistical or analytical materials, or any reports based on information obtained from studies covered by this contract, will be subject to review and approval by the COR before publication or dissemination. 7.7 Conflict of Interest. The contractor shall not hire any employee for performance under this contract whose employment would result in a conflict with Government of Navy Standards of Conduct.
