STATEMENT OF WORK Worship services - muslim inmates Federal Correctional Institution Seagoville, Texas PART I Section A - Introduction The Federal Correctional Institution, Seagoville, Texas, combining this synopsis/solicitation for a non-personal services contract to provide professional Islamic Imam duties for Muslim male inmates incarcerated at the Federal Correctional Institution, Federal Detention Center, and Federal Prison Camp. Please be advised this is a contractual position only and is not to be construed to be a personnel appointment to the Federal Bureau of Prisons. The anticipated period of performance is from 10/01/2004 thru 9/30/2005 with four (4) one year options to be exercised at the discretion of the government. RFQ 51402-009-5 is issued as a request for quotation. The solicitation document and incorporated provisions are those in effect through Federal Acquisition Circular 2001-24. This solicitation is set-a-side for small business. The government intends to make a single award with a firm fixed price. Award will be made based on price and past performance. Section B - Price Quotes 1. $_________________ per session ( 1.5 hours ) 90 minutes for fiscal year 2005 2. $_________________ TOTAL ( approx. 158 sessions ) for fiscal year 2005 (BASE YR) 3. $_________________ per session (1.5 hours) 90 minutes for fiscal year 2006 4. $_________________ TOTAL ( approx. 158 sessions ) for fiscal year 2006 (OPTION 1) 5. $_________________ per session ( 1.5 hours ) 90 minutes for fiscal year 2007 6. $_________________ TOTAL ( approx. 158 sessions ) for fiscal year 2007 (OPTION 2) 7. $_________________ per session ( 1.5 hours ) 90 minutes for fiscal year 2008 8. $_________________TOTAL ( approx. 158 sessions ) for fiscal year 2008 (OPTION 3) 9. $_________________ per session (1.5 hours ) 90 minutes for fiscal year 2009 10. $________________TOTAL ( approx. 158 sessions ) for fiscal year 2009 (OPTION 4) Section B - Scope of Work The successful contractor will be required to provide worship services for Muslim inmates and to aid in the design and structuring of the ongoing Islamic faith program at the Bureau Facilities in Seagoville, Texas. The contractor will organize and plan Islamic feasts, conduct weekly Jumah prayer services and programs for inmates; instruct Islamic inmates in Quranic studies and life skills; and provide pastoral care and counseling to Islamic inmates. Section C - Applicable Documents The Contractor will adhere to all regulations prescribed by the Department of Justice (DOJ), Bureau of Prisons (BOP), and Federal Correctional Institution (FCI), Seagoville, Texas, for the safety, custody, and care of inmates. A knowledge of current BOP policies and procedures is essential for effective performance of this service. Section D - Technical Work Requirements Provisions of service under this contract will require frequent and unsupervised contact with inmates. The contractor will not be responsible for the management of inmates other than to ensure that those inmates involved in activities within the contractor’s scope of work shall abide by all rules in effect to ensure their safety and well being. The Contractor will monitor attendance of inmates and will report infractions of policy or procedures to the Staff Chaplain. The contractor will adhere to all freedom of information and privacy procedures that are incorporated by the contract. The Contractor will provide written reports as may be required by the Supervisor of the Department of Pastoral Care. These reports may be in handwritten or typewritten form, verified by the contractor’s signature. Sections E - Supplies All supplies and books will be provided by the Bureau Department of Pastoral Care. No materials, supplies, or equipment will be brought into the institution without the prior written approval of the Supervisor of the Department of Pastoral Care. The contractor will be expected to account for all materials, supplies and equipment placed under his/her disposition. PART II Section A - Period / Schedule of Performance The services provided, being professional in nature, will be provided in the method considered normal in the local community. These services, although not directly supervised, will be reviewed by the Supervisor of the Department of Pastoral Care. The performance of these services will also be verified by the designated Contracting Officer’s Technical Representative (COTR). The COTR shall ensure compliance with all aspects of the Statement of Work (SOW). It is estimated the contractor will work approximately (158) sessions per fiscal year. One session shall be a period of one and one-half (1 ½ ) continuous hours. Sessions shall be performed at a mutually agreed upon time in accordance with Department of Pastoral Care. The contract will not exceed 30 hours per week. The period of the contract shall extend from Date of Award through September 30, 2005 with four (4) one year options to be exercised at the discretion of the government. Section B - Place of Performance Services will be performed on-site at the Federal Bureau of Prison Facilities consisting of the Federal Correctional Institution/Federal Detention Center and Federal Prison Camp located at 2113 N. Highway 175, Seagoville, Texas 75159-2237. Section C - Contractor Qualifications The Contractor shall be educated and trained for vocation of Imam in Islamic masjid and endorsed by an appropriate religious body. Offerors are requested to submit photocopies of credentials with the submission a price quote. The Contractor shall provide FCI, Seagoville, with a complete resume and “Application for Federal Employment - OF 612" application packet prior to contract award. PART III Sections A - Security Requirements The following investigative procedures will be required in accordance with the Bureau of Prisons Program Statement 3000.02; Chapter 731.2, Pages 35-41, dated November 1, 1993, and Operations Memorandum 157-93 (5500) dated August 18, 1993. The contractor must also be in compliance with DOJ regulations at 8 C.F.R. 274a concerning the use of illegal aliens on this contract. The Supervisor of Department of Pastoral Care for the institution where you as a Contractor are to provide services, is responsible for conducting the appropriate vouchering, law enforcement checks, and ensuring fingerprinting is completed on yourself and all personnel directly or indirectly employed by you that may need access inside the confines of the perimeter. The Personnel Department will establish a security file that will be maintained on each contractor and/or their employees from the beginning of the work until the duration. The following information will be maintained in this file: 1. National Crime Information Center (NCIC) check; 2. DOJ-99 (name check) 3. FD-99 (fingerprint check); 4. Law Enforcement Agency checks; 5. Vouchering of Employers over past five years; 6. SF-171 (Personnel Qualifications Statement) and/or an OF-612 (Application for Federal Employment) 7. OPM-329-A (Authority for Release of Information); and 8. National Agency Check and Inquiries (NACI) check (if applicable). Each prospective contractor is advised a urinalysis test is required (for detection of drug and marijuana usage) before the contract is awarded. If the proposed contract is with a company and the individual(s) assigned to perform the work test positive, the individual(s) shall be excluded from performing the contract and the contractor shall provide acceptable replacement personnel subject to the same security requirements. If the proposed contract is with an individual, the contract award may not be made until after a negative test is received. The Contractor will be required to participate in an orientation session for Contractors provided by the Bureau Facilities at Seagoville, Texas. Any individual deemed not suitable by Bureau of Prisons requirement, will not be granted access to work within the confines of perimeter security. This is a condition of the contract. Section B - Supervision contract administration: (a) Contracting Officer responsibility - Authority to negotiate changes in terms, conditions, or amounts cited in this contract is reserved to the Contracting Officer. (b) Contracting Officer’s Technical Representative (COTR) - the Contracting Officer will designate in writing a COTR who shall be responsible for: (1) Monitoring the technical progress on individual task orders, including surveillance and assessment of performance and issuing technical changes (2) Interpreting the Scope of Work; (3) Technical evaluation as required; (4) Technical inspection and acceptance; (5) Assisting in the resolution of technical problems encountered by the service provider during contract performance; (6) Monitoring funds available for obligation; (7) Ensuring that invoices and payments are processed in a timely manner; and (8) Assisting in resolution of administrative problems encountered by the service provider during contract performance. Attachment I DEPARTMENT OF JUSTICE CONTRACTOR RESIDENCY REQUIREMENT BUREAU OF PRISONS CLAUSE The residency requirement applies to both individuals and employees of companies providing services to the Federal Bureau of Prisons. For three of the five years immediately prior to submission of your offer/bid/quote, individuals or contractor employees providing services must have: 1. resided in the United States (U.S.); 2. worked for the U.S. overseas in a Federal or military capacity; or 3. been a dependent of a Federal or military employee serving overseas. ------------------------------------------------------------------------------------------------------------------------------------------- Department of Justice Residency Requirement Certification Form The residency requirement certification must be completed and returned with the offer/bid/quote for individuals applying for a contract.
