Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued.PERFORMANCE WORK STATEMENT. CLINICAL AND SOCIAL SERVICES ASSISTANT C.1. GENERAL. The purpose of the Clinical and Social Services Assistant is to assist commanders, soldiers and family members with problems of child abuse/neglect and spouse abuse. Work is to be performed in support of the Fort McPherson Family Advocacy Program in Behavioral Science Service of the Lawrence Joel Army Health Clinic and to assist other FAP treatment providers with administrative duties. Duties include clinical and administrative functions and require a background in mental health and compet ence in computer technology to maintain automated records and client data systems. Experience in clinical interviewing and report-writing is essential. This is a personal services contract, and the contractor works under supervision of the Chief of Behav ioral Science Service at LJAHC.C.1.1. SCOPE OF WORK. C.1.1.1 The contractor manages a case load of approximately 40 child/spouse abuse cases, discussing cases and unusual problems with the supervisor and/or treatment provider to ensure proper assessment and disposition. May be required to provide resources to persons in emergency need, as well as to handle routine referrals. C.1.1.2. Under supervision, conducts initial interviews of family and collateral contacts and reviews records of abusing/neglecting families to obtain information for a social and psychological history, along with other medical, work, and personal data. Uses the information obtained to consult with credentialed providers about cases and then to make critical recommendations regarding safety of children and spouses. Work may require contact with court/local child protective services. With credentialed pr ovider, develops a case assessment to identify family problems, contributing to child/spouse abuse dynamic. Reports all suspected cases of child abuse/neglect to the unit commander, county child protective service and/or law enforcement agencies. Records information and participates with a credentialed provider in preparing a proposed, problem-based treatment plan for each case for presentation to the Case Review Committee (CRC). Arranges for social work, pediatric, drug and alcohol, and psychological an d psychiatric evaluations of patients as required. Communicates with other agencies as deemed appropriate, i.e., physicians, Criminal Investigation Division (CID), county departments of family and children services, commanders, etc., receiving and providi ng collateral information. C.1.1.3. Under supervision, conducts interviews with children, parents, spouses, to determine problems and develop recommendations for resolution. Responsible for maintaining a helpful relationship with parents, children, spou ses, through interviews, limited short-term support and referral. C.1.1.4. Maintains accurate automated client records of all contacts (face-to-face and telephone). Obtains information from various agencies and clinics and provides the same in order to e valuate and protect individuals involved in domestic violence situations. Prepares automated reports of suspected abuse and notifies appropriate agencies in a timely manner.C.1.1.5. Maintains liaison with local child protective services, medical staff, c ommanders, civilian agencies, and members of CRC involved in case evaluation and treatment to ascertain progress. Reports progress and newly identified needs to CRC to ensure that all are aware of the goals and actions of others so a unified approach will occur. Briefs the provider/supervisor on any new developments that may impact on the case/CRC recommendations. Under supervision of a credentialed prov ider, may serve as therapist assistant for Levels I and II treatment, providing support and education to individuals, families, and groups. Assures command awareness and involvement with each case throughout the CRC process. C.1.1.6. Participates in staff development programs. Maintains close working relationship with other members of the FAP staff by attendance at professional-administrative staff meetings and conferences with staff consultants. C.1.1.7. Performs administrative duties in support of case documentation (i.e., preparation of forms and reports) as required under AR 608-18 and local operations. C.1.1.8. Performs other duties as assigned and as permitted through credentialing or other agreement. C.1.2. PERSONNEL. Contractor shall have minimum of bachelor's degree, preferably in psychology or social work. Preference will be given to those with work experience and/or training in health care setting or social service agency. Must have competence in office automation equipment and use of word processing and spreadsheet software, as well as the ability to learn other software packages that may beco me necessary for successful completion tasks. C.1.2.1. ARMY CENTRAL REGISTRY BACKGROUND CHECK. Contractor shall not be listed as an offender in the Army Central Registry (ACR), which contains certain data about those connected with incidents of domestic violence.C.1.3. SUPERVISORY CONTROLS. Works under the general supervision of the Chief, Behavioral Science Service, LJAHC. Assignments are made in terms of objectives to be achieved with suggestions for accomplishment. Contractor shall plan and carry o ut the work in accordance with program objectives and policy and promptly refers serious matters (e.g., high risk or threat to life) to supervisor and/or credentialed provider for discussion and resolution. Work will be reviewed for progress through perio dic discussions of cases, review of records and reports.C.1.4. SECURITY REQUIREMENTS. The contractor has no access to documents or information with a higher security classification than For Official use Only. C.1.5. CONFIDENTIALITY. Strict confidentia lity shall be maintained regarding personal information of clients. Client records will be maintained in accordance with existing departmental policy, which is IAW AR 608-18. In general, records shall be maintained in a restricted file and locked during those times when staff authorized access are not present. Without written consent and request, only general information should be released to commanding officers in cases, (i.e., cooperation, progress, and failure to keep appointments), unless otherwise r equired and authorized by regulation and policy.C.1.6. QUALITY CONTROL. Contractor will participate in quality control activity as requested, to support local and other Performance Improvement (PI) programs. C.1.7. PHYSICAL SECURITY. Contractor shall establish and implement methods to ensure the physical security of all Government property and facilities. Methods established shall be in accordance with locally directed physical security and internal control procedures. Access to facilities shall be l imited to those whom the local commander has authorized. C.1.8. SAFETY. Contractor shall be aware of local safety requirements and shall make reasonable efforts to protect persons from harm and Government property from damage. C.1.9. EDUCATION. Contractor to receive training at Government expense only in accord with the contract. Family Advocacy Staff Training (FAST course) provided at no cost to contractor, scheduled by Government agency at its convenience with travel funded , if necessary and per diem costs in accordance with joint travel regulation current at time of travel arrangements. Failure to attend and complete determined training will result in loss of position of contractor. Premature departure of contractor after completing FAST obligates contractor to reimburse the Government for the cost of education, TDY, and per diem. C.1.10. NORMAL WORKING HOURS. The Contractor shall perform the tasks of this contract during normal duty hours (7:30 a.m. to 4:15 p.m.), Monday through Friday, unless other hours are approved by the supervisor.C.3. GOVERNMENT FURNISHED PROPERTY. C.3.1. The Government will provide to the contractor for use in connection with this contract the following property:C.3.1.1. Building--shared office space for one contractor.C.3.1.2. Telephone--shared with other staff. C.3.1.3. Heat, light and other utilities. C.3.1.4. One computer and one printer for contractor utilization, as well as the sharing of other office equipment and supplies as needed and available.C.3.1.5. Standard Army and Government forms and regulations as required and requested. C.3.1.6. Office furniture, to include desk and chair.C.3.1.7. Printing of special forms as needed and approved for use by printing and publications office.C.3.2. The Government property, made available under this contract, shall be for use only in the p erformance of the contract.C.3.3. The Government will not be responsible in any way for damage to the contractor's supplies, materials, equipment, or personal belongings brought into the building or onto the grounds due to fire, theft, accident, or other disaster.C.3.4. Inventory of property will be prescribed by relevant regulation. The contractor will be liable for all shortages resulting from contractor negligence or imprudence. C.4. CONTRACTOR FURNISHED PROPERTY. The contractor shall not be requi red to furnish any property or materials relevant to this contract but shall be responsible for receiving, caring and accounting for returning any Government furnished property. The contractor should familiarize him/herself with proper care and use of Gove rnment property. The operation of any necessary equipment, to include training aides within the services to be delivered hereunder, shall be at no additional expense to the Government over and above the contract price.C.5. SPECIFIC TASKS. C.5.1. Under supervision of credentialed provider, interviews individuals referred for evaluation IAW AR 608-18; with supervisor or provider, assesses potential risk and makes appropriate recommendations and referrals to prevent harm.C.5.2. Gathers information and mak es referrals for the purpose of gathering further information. C.5.3. Under supervision and with the FAP social worker or other credentialed provider, organizes case information and prepares presentations for the Case Review Committee (CRC). Presents cas es along with a recommendation regarding a case determination, IAW AR 608-18, the Child Abuse Manual (CHAM), the Spouse Abuse Manual (SPAM), and local operations, with the understanding that determination and treatment recommendations are those of credenti aled providers.C.5.4. Following CRC meetings, records proceedings (IAW with local operations) that will become part of official CRC minutes. After official approval of the minutes, assures that case proceedings become part of the case file. C.5.5. Foll owing CRC meetings, assures that DD Forms 2486-R are completed and submitted to the Central Registry IAW AR 608-18, and performs other tasks that are part of local operations of the CRC.C.5.6.
