Description
The New Mexico VA Health Care System (NMVAHCS) intends to negotiate a Commercial (FAR Part 12), (Simplified Acquisition Procedures FAR 13) firm-fixed priced, Indefinite Delivery Indefinite Quantities, non-personal services contract for Health Care Resources in accordance with Public Law 104-262 and the authority of Title 38 USC Section 8153 through a request for Proposal for a non-restrictive solicitation. The NMVAHCS, 1501 San Pedro Blvd SE, Albuquerque, NM 87108, is seeking for a qualified contractor that can perform Routine and Stat Laboratory Test on patients or specimens referred by 6 NMVAHCS Community Based Outpatient Clinics(CBOC) located throughout the state of New Mexico. The facilities to be serviced are located in Artesia, Farmington, Gallup, Raton, Santa Fe, and Silver City, New Mexico The Contractor will provide personnel, facilities, supplies, and equipment necessary to perform the required test which include but are not limited to: 1. Obtaining specimen(s) for laboratory test(s), and/or 2. Perform laboratory tests on specimen(s). Contractor facility must be located within a ten (10) mile radius of the CBOC for the convenience of the beneficiaries. The contract period will be for one (1) base year from date of award with three (3) one (1) year options to renew. This request is being advertised as unrestrictive with full and open competition and multiple awards will be allowed. NAICS Code 621512. Electronic copies of the solicitation will be available at no cost on or about March 28, 2005 at the following URL: http://www.bos.oamm.va.gov/cgi-bin/WebObjects/BosPublic.woa. Any amendments to this solicitation will be posted by the VA without notice, and will be available at this same URL address. Offerors are advised that they are responsible for obtaining any and all amendments. No web-based offers will be accepted. All questions must be in writing and may be faxed to (505) 248-3206. No telephone request will be honored. The estimated cost range is $250,000-$500,000.
