Description

The Minnesota National Guard, through the USPFO for Minnesota, has a requirement for On-Site Dental Examinations. The testing will be required at various locations through-out Minnesota. Dental Exams required but not limited to the following service s: Complete Exam; Exam with Bitewing; Exam with Bitewing and Pano; X-ray, bite wing, single film; X-ray, two bite; X-ray, Panorex; X-ray, complete series. X-ray items must provide digital files on CDROM. The contractor must provide the following personne l: Dentist; Dental Hygienist; and Dental Technician. The contract period will be from date of award through the end of fiscal year 2005 (30 SEP 2005). This will be an unrestricted acquisition, not set-aside for any specific group. All firms may apply, a nd submissions from small business and small disadvantaged business are encouraged. Request For Proposal (RFP) will be available o/a 11 April 2005 with proposal due date o/a 20 April 2005.
