- THIS AMENDMENT IS ISSUED TO MAKE THE FOLLOWING CORRECTION TO THE CUTTER'S NAME AND LOCATION. (**SENTENCE 8**)ALL OTHER TERMS AND CONDITIONS REMAIN UNCHANGED. This is a combined synopsis/solicitation for commercial item prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation WILL NOT be issued. The Request for Quotation number is HSCG89-05-Q-632036 and incorporates provisions and clauses in effect through Federal Acquisition Circular 01-26. This acquisition is 100% small business set-aside. The NAICS is 238330. The small business sized standard is $6M annually. The contract type will be a firm fixed price purchase order. The Government proposes to solicit quotes for the replacement of Electrical Safety Matting The Government proposes to solicit quotes for the replacement of Electrical Safety Matting on board **USCGC Midgett (WHEC-726 at her homeport moorings located at Pier 36, 1519 Alaska Way South, Seattle WA 98134**. SCOPE: Contractor will provide all materials, equipment and labor, to remove and dispose of all existing electrical safety matting in spaces identified. Repair and patch underlayment as necessary. Install new blue marbled electrical safety matting. Matting used must comply with MIL-M-15562G (copy available upon request). All other materials must be designated for use in shipboard environments. Work to be accomplished onboard USCGC Midgett (WHEC-726) at her homeport moorings located at: Pier 36, 1519 Alaskan Way South, Seattle, WA 98134. Estimated work to be accomplished in the following spaces (actual spaces and square footage to be determined during ship check). a. Electronics Shop - 200sf b. Transmitter Room - 280sf c. Electronics Room ? 310sf d. Radio Room ? 420sf e. CIC Room ? 700sf f. CWIS Room ? 165sf g. MK92 control room ? 40sf h. Aft ESM ? 100sf i. ELEX II ? 150sf The period of performance to be determined. The contractor will be responsible for removing all equipment and tools as each space is completed. Contractor is also responsible for removing any glue or residue left on the matting after installation. Vessel will be available for a walk through 18 April through 23 April 2005.
