Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This is solicitation number HQ0423-05-T-0009: The solicitation is issued as a request for quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular, FAC 2001-26, Effective 20 Dec 04. This requirement is 100% set-aside for small business. The NAICS code is 238160 and the small business size standard is $12 million. Line Item Items Quantities UOM 0001 Roofing Repair-see SOW for details 1 Each Description of items/services: STATEMENT OF WORK (SOW) Cut and remove existing damage roof down to metal decking Install 2 layers of polyisocyanurate insulation (2? and 1 ??) Mechanically fastened down to the metal decking with screws and metal plates Fully adhere a .060 Carlisle EPDM roof according to manufacturers specifications Tie in new roof to existing roof according to manufacturers specifications New roof terminated over gutter edge with aluminum and fastened Area will parapet walls-new roof will extend up one side with existing metal coping reinstalled Dispose of all material removed from the old roof Ensure work area is cleaned from any remaining debris Warranty on material and workmanship Prime and scal all seams IAW manufacturers specifications. Total Square Footage to be repaired and replaced is 6,090 Sq. Ft. Work hours 7:00 a.m.- 4:00 p.m. Monday-Friday (not including Government holidays). Site foreman will sign all employees in and out daily at the security desk. Contractor ID card must be worn at all times while in the building. Work area must be cleaned daily. Delivery date: 20 Days after Date of Award. Place of Performance is Dayton, OH. FOB Destination. The provision at 52.212-1, Instructions to Offerors -- Commercial, applies to this acquisition. FAR 52.212-2, Evaluation -- Commercial Items, is applicable. As prescribed in 12.301(c), the Contracting Officer may insert a provision substantially as follows: Evaluation -- Commercial Items (Jan 1999) (a) The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers: Price and Past Performance. (b) A written notice of award or acceptance of an offer, mailed or otherwise furnished to the successful offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party. Before the offer?s specified expiration time, the Government may accept an offer (or part of an offer), whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award. Offerors are to include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications -- Commercial Items, with their offer. The clause at 52.212-4, Contract Terms and Conditions -- Commercial Items, applies to this acquisition. The clause at 52.212-5, Contract Terms and Conditions Required To Implement Statutes Or Executive Orders -- Commercial Items, applies to this acquisition. Any additional contract requirement(s) or terms and conditions: The Davis Bacon Act applies. The Defense Priorities and Allocations System (DPAS) applies and the assigned rating for this requirement is DOC9. Offers are due by 1:00 p.m. EST on 02 May 2005.
