Description
The Central Texas Veterans Healthcare System is seeking sources to provide services in accordance with the statement of work (see below). Please contact Glenn Caskey at 254-743-2243 or joseph.caskey@med.va.gov with information or questions. STATEMENT OF WORK 1. Term of contract: The equipment listed below will be rented by the Central Texas Veterans Health Care System (CTVHCS). This rental agreement will be in effect from date of award through 12 months and may include option years. The initial award shall be for 60 cases/year, after which the government will determine whether to extend the rental agreement. 2. Equipment: -80-watt Holmium YAG surgical laser system -Laser safety eyewear (10+ pairs) -Laser signage (2+, wavelength specific) -Laser fibers for urological and arthroscopic procedures -Certified, factory-trained laser operator -Delivery, installation and set-up -Current preventive maintenance verification by laser system manufacturer -Registration of all equipment with the Bureau of Radiation Control The laser operator provided by the contractor shall be responsible for: 1. Deliver the laser and have it checked by Biomedical Engineers 2. Hang/retrieve laser danger signs 3. Distribute/retrieve protective eyewear to personnel in OR suite 4. Operate the laser per the surgeon?s instructions 5. Acquire purchase order number (if applicable) 6. Load laser 3. Liability: The contractor shall maintain throughout the term of this contract, liability insurance covering its activities under the contract in at least such amount(s) as is required by applicable laws and regulations. The contractor shall furnish evidence of such insurance prior to contract award. The contractor will be liable for the safety and performance of the rental equipment provided. The contractor shall be solely liable for and expressly agrees to indemnify the Government with respect to any liability producing acts or omissions by it, its employees or agents. The contractor shall maintain during the term of this contract liability insurance issued by a responsible insurance carrier not less than $1,000,000.00 per occurrence. The contractor agrees to maintain such a policy for the duration of this agreement. 4. Service: The contractor agrees to provide all services described in this rental agreement. This service and its documentation will meet or exceed JCAHO requirements for this type of equipment. The contractor assumes no responsibility for any condition affecting proper operation of the equipment resulting from accident, negligence, abuse or alterations made to the equipment by anyone other than a representative authorized by the contractor. 5. Terms of Payment: Payment terms are net 30 upon receipt of proper invoice. 6. This agreement may be terminated without cause by either party with a sixty (60) day written notification.
