Description
POC: Nicasio C. Arabe Tel: (661) 277-8777) SUBPART: F999- Other Environmental Services. This amendment is issued to include to the Combined Synopsis/Solicitation the following STATEMENT OF WORK(SOW): 1. DESCRIPTION OF SERVICES NON-PERSONAL. The contractor shall provide all labor, tools, material, equipment and transportation necessary to remove industrial waste and small quantity of liquid from a 80’X70’X2’ evaporation pond located at the Air Force Research Laboratory at Edwards AFB, CA. 1.1. BASIC SERVICES. The contractor shall remove approximately 1.3-1.5 million pounds of a salt based solid and liquid from the South Evaporation Pond and dispose of the industrial waste at a certified disposal facility in accordance with federal, state and local laws. 1.1.1. During the removal of the solid waste, plastic tools are to be used so as not to damage the high-density polyethylene liner. 1.1.2. All solid/liquid waste removed from the evaporation ponds will be weighed at the AFRL scales prior to transporting to the disposal site. 1.1.3 Upon removal of the solid/liquid industrial waste the liner is to be swept and rinsed of any residual. 1.1.4. The contractor shall comply with all federal, state and local laws pertaining to safety, health, environmental compliance and transportation of it’s workers, subcontractors and waste during the performance of the cleaning. 1.1.5 Perform tasks necessary to maintain general cleanliness and safety of the work area. 2. SERVICE DELIVERY SUMMARY Performance Objective SOWPara Performance Threshold All solid/liquid waste removed from the evaporation ponds will be weighed at the AFRL scales prior to transporting to the disposal site. 1.1.2 To be performed 100% each load. Upon removal of the solid/liquid industrial waste the liner is to be swept and rinsed of any residual. 1.1.3 To be performed 95% of the time 3. GOVERNMENT FURNISHED PROPERTY AND SERVICES. The following is a list of Government furnished equipment: Portable Office measuring 6’X12’ with air conditioning Emergency Eye Wash station 1 ˝” Fire Hose Calibrated weight scale facility 4. GENERAL INFORMATION: 4.1. QUALITY ASSURANCE. The government will periodically evaluate the contractor’s performance by appointing a representative(s) to monitor performance to ensure services are received. The government representative will evaluate the contractor’s performance through intermittent on-site inspections and receipt of complaints from AFRL personnel. The government may inspect each task as completed or increase the number of quality control inspections if deemed appropriate because of repeated failures discovered during quality control inspections or because of repeated customer complaints. Likewise, the government may decrease the number of quality control inspections if performance dictates. The government will also receive and investigate complaints from various customers located on the installation. The contractor shall be responsible for initially validating customer complaints. However, the government representative shall make final determination of the validity of customer complaint(s) in cases of disagreement with customer(s). 4.2. GOVERNMENT REMEDIES. The contracting officer shall follow the requirements of FAR 52.212.-4, Contract Terms and Conditions for Commercial Items (Oct 2003), for contractor’s failure to correct nonconforming services. 4.3. HOURS OF OPERATION. Normal work hours shall be agreed upon prior to contract start date and remain flexible throughout the contract pending day-today temperatures. Work hours may contain more then one shift as specified in the contractor’s proposal and will not exceed the contract award amount. Due to the nature of laboratory activities, some operations may incur delays prolonging completion of the prescribed work. The contractor shall normally be notified in advance at the earliest opportunity of any delays causing a work stoppage. 4.4. SECURITY REQUIREMENTS. The contractor shall provide all personal information necessary for all scheduled workers under the prime and/or any sub contractor that needs access to the AFRL for the purpose of performing this contract. The Prime contractor shall provide the full name, SSAN and date of birth for all personnel and must ensure that all personnel are US and/or Naturalized Citizens. Naturalized citizens must show proof of their naturalization. Workers will be placed on a daily access list and will proceed directly to/from the work site. 4.6. WORK UNIFORMS. The employees shall wear all required clothing articles necessary in the removal of industrial waste in accordance with federal, state and local laws.
