Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number N68836-05-T-0098 is issued as a request for quote (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through the lastest edition of the Federal Acquisition Circular. This solicitation is issued on an total Small Business Set-A-Side basis, (see numbered note 1). The following clauses and provisions apply: 52.212-1 Instructions to Offerors-Commercial Items;52.212-3 Offeror Representations and Certifications-Commercial Items; 52.204-7 Central Contractor Registration; 52.212-4 Contract Terms and Conditions-Commercial Items; 52.212-5 Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items, and citing: 52.222-21, 52.222-26, 52.222-35, 52.222-36, 52.222-37, 52.225-13, and 52,232-36; 252.204-7004 Central Contractor Registration; 252.212-7001 Contract Terms and Conditions Required to Implement Statutes Applicable to Defense Acquisition of Commercial Items, and citing: 252.225-7001 and 252.232-7003; Offers are due by close of business 12 May 2005 and may be emailed to Carolyn.wilson1@navy.mil or faxed to 904/270-6498. Point of Contact is Carolyn Wilson, Ph 904/270-5699 x147 STATEMENT OF WORK: 1. General: This statement of work provides for material and services required for refurbishment of the Commanding Officer ?At-Sea-Cabin? and Executive Office/Stateroom/Supply Office onboard USS Vicksburg CG-69. 2. Tasks: The contractor shall on the terms and conditions more particularly described herein, furnish the management, labor, and material necessary and incidental to the performance of work set forth below. 2.1 Period of Performance: June 06 2005 through June 16 2005 (CMAV) 3. CO At-Sea-Cabin Specifications: 3.1 Corner Sofa. Re-upholster existing corner (four piece sectional) sofa unit. The sofa has eight (8) cushions with tight back. The fabric covering shall adhere to specifications set forth in NAVSEA habitability material list. Ship?s force will select fabric color. Remove sectional sofa to contractor?s facilities for reupholster, strip sofa, saving old fabric as template for new selected fabric, install new padding, cushioning. Reinstall sofa unit. Repair and paint all surfaces disturbed during renovations or improvements. 3.2 Carpet. Remove the existing floor covering in office and berthing space. Area size to be covered in approximately 38 square yards (vendor responsible for measuring space), ship?s force will select fabric color. Discard old carpet covering, clean epoxy adhesive, repair, and prepare deck surface as needed for new carpet. Install new carpet that meets Fed Spec DDD-C-95 Type II Class I in both spaces. Repair and paint all surfaces disturbed during renovations or improvements. 4. Executive Officer Office/Stateroom Specifications: 4.1 Transom Sleeper Sofa. Re-upholster existing transom/sofa unit (seat, back, and arms). The fabric covering shall adhere to specifications set forth in NAVSEA habitability material list. Ship?s force will select fabric color. Remove sectional transom/sofa to contractor?s facilities for reupholster-strip transom/sofa, saving old fabric as template for new (selected) fabric, install new padding, cushioning. Reinstall transom/sofa unit. Repair and paint all surfaces disturbed during renovations or improvements. 4.2 Computer Work Station/Desk. Modify existing workstation (90?w x 76?h x 36?d) surface with new laminate (HPPL). Remove workstation, to include cabinet drawers, doors, secretary station, keyboard trays and divider bars to contractor?s facilities to strip existing laminate from surface. Clean epoxy adhesive and prepare surface for laminate. Re-laminate, with ship?s force color selection. Paint all uncovered surface area to match laminate. Reinstall workstation unit. Repair and paint all surfaces disturbed during renovations or improvements. 4.3 Storage Cabinet. Modify existing cabinet (74?w x 31?h x 13?d) surface with new laminate (HPPL). Remove cabinet, to include cabinet doors, and divider bars to contractor?s facilities to strip existing laminate from surface. Clean epoxy adhesive and prepare surface for laminate. Re-laminate, with ship?s force color selection. Paint all uncovered surface area to match laminate. Reinstall cabinet. Repair and paint all surfaces disturbed during renovations or improvements. 4.4 Entertainment Cabinet. Modify existing cabinet (36?w x 31?h x 30?d) surface with new laminate (HPPL). Remove cabinet, to include cabinet doors, and divider bars to contractor?s facilities to strip existing laminate from surface. Clean epoxy adhesive and prepare surface for laminate. Re-laminate, with ship?s force color selection. Paint all uncovered surface area to match laminate. Reinstall cabinet. Repair and paint all surfaces disturbed during renovations or improvements. 4.5 Fabric Wall Cover. Remove existing fabric bulkhead wall cover surface (106? x 92?). Discard old wall covering. Prepare surface and install new ship?s force color and fabric selection. Paint all trim surfaces not being covered with fabric wall cover. 4.6 Deck. Remove existing carpet floor covering in office/stateroom. Area size to be covered is approximately 162 square yards, ship?s force will select tile color. Discard old carpet covering, clean epoxy adhesive, repair, level, and prepare deck surface for vinyl tile. Reinstall Azrock vinyl tile. Modify, manufacture, install, and seal new custom fit transom/locker floor edges after completion of tiling. Repair and paint all surfaces disturbed during renovations or improvements. 5. Supply Office Specifications: 5.1 Refurbish, modify, and install new shipboard approved office furnishing in supply office per list. All furniture will be heavy-duty welded aluminum with laminate (HPPL) exteriors. Adhere to all applicable specifications set forth in NAVSEA habitability materials list. 5.2 Storekeeper Work Station. Remove existing L shape workstation table and storage cabinet. Discard old table and cabinet. Modify present deck mounts for installation of new workstation. Repair and paint all surfaces disturbed during renovations or improvements. Manufacture and install 3-person station work center to consist of: a. 1 each 28? x 156? L shape counter top b. 3 each CPU tower pedestals with utility drawer c. 1 each 3 drawer letter size file cabinet d. 1 each modular wall mounted bookcase to include: 3 each 14?H x 14?D x 52?L bookcase. Bookcases will have hinged doors and single row routing boxes mounted below. 5.3 3MC Work Station: Remove existing workstation and storage cabinet. Discard old workstation and cabinet. Modify present deck mounts for installation of new workstations. Repair and paint all surfaces disturbed during renovations or improvements. Manufacture and install 1 station work center to consist of: a. 1 each 28? x 72? counter top b. 1 each CPU tower pedestals with utility drawer c. 1 each 3 drawer letter size file cabinet d. 1 each modular wall mounted bookcase to include: 14?H x 14?D x 52?L bookcase. Bookcases will have hinged doors and single row routing boxes mounted below. 6. Ships Force Responsibilities: 6.1 Remove and reinstall all ships service systems and interference (i.e. steam, potable water, drains, AC, electrical and IC, etc.) 6.2 Provide permission to perform hot work with applicable hot work chits, gas free certificates and fire watches. 6.3 Provide ships representative liaison to facilitate and speed of any and all contractor/ship coordination and limit delays and work. 6.4 Secure and limit traffic to work area to allow timely and professional performance by the contractor. 6.5 All services required not listed to perform scope of work. 6.6 Any and all additions, subtractions and modifications to ships systems (i.e. steam, potable water, drains, AC, electrical and IC, etc.) 6.7 Cover and protect interference and furnishings not being removed during renovations or improvements. 6.8 Ensure adequate electrical power, LP air, access, and materials storage aboard ship and ventilation 7. Habitability Requirements: 7.1 All work to be in accordance with applicable specifications as set forth in NAVSEA habitability materials list, revision K, dated May of 1996
