Description
VA Medical Center (VAMC), Minneapolis, MN solicits proposals for Elevator Improvements prepared in accordance with the format in FAR Subpart 13.106, as supplemented with additional information included in this notice. Solicitation #RFP 618-141-05 is assigned as Request For Proposal (RFP) and may be downloaded from the website at http://www.bos.oamm.va.gov or are available upon written request (fax 612-725-2072). The solicitation documents and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 05-03. This procurement is issued as a set-aside for small business ? NAICS code is 561790 and the small business size standard is $6 million. Line Item #1: 14 each - Provide all labor and necessary material to install shunt trip protection to main line disconnects in the machine rooms of A Bank (4 ea), B Bank (4 ea), C Bank (3 ea), G Bank (2 ea) and M11 (1 ea) Elevators; to include an I/O from fire fighters service to indicate the elevators are at the main floor with doors open before the sprinkler system is activated (ASME 17.1 Rule 102.2(c)3. Line Item #2: 4 each - Provide all labor and necessary material to install Electronic Door Protection Device on both front and rear doors of Elevators in Bldg. 9 and Bldg. 10; the Electronic Door Protection Device shall be equal to or better than the Formula Systems Safescreen FCU47 and be ADA and ANSI A17.1 compliant. Line Item #3: 1 job - Provide all labor and necessary material to install two-way communication system between Elevators and Medical Center?s designated dispatch center (Telephone Operators) in Bldg. 49 Elevators S1, S2 and S3 (total of 3) and shall be ADA and ANSI A17.1 compliant; the means of communication shall be a ?Talk-A-Phone? (or equal) hands free with built-in dialer. Line Item #4: 1 job - Provide all labor and necessary material to install Safety Bar(s) in hoistway roof vent for Elevator S3, Bldg. 49 in accordance with NFPA 72. Line Item #5:. 1 job - Provide all labor and necessary material to install Pit Ladder in Dumbwaiter M3. Add Alternate Line Item #6: (7 ea) - Provide shunt trip protection and I/O as in Line Item #1 for Elevators D Bank (3 ea), E Bank (3 ea), and M1 (1 ea), Bldg. 70. Contractor must provide own tools, equipment and incidental materials for installation; perform daily general cleanup. Complete all work within 30 calendar days of issuance of Award/Notice to Proceed, exclusive of holidays, Monday through Friday, during normal working hours from 8:00 a.m. to 4:30 p.m. Award will be for a firm fixed-price contract, FOB destination with payment in arrears on certified invoice. A pre-bid site walk is scheduled for Tuesday, May 17, 2005, at 9:00 a.m. local time. Meet in Facilities Engineering, Room BR 138, Bldg. 70, VA Medical Center, One Veterans Drive, Minneapolis, MN 55417. Failure to attend this pre-bid conference will not relieve the successful Contractor from compliance with all specifications, terms and conditions of the resultant contract. Service Contract applies and Wage Determination #1994-2287 (Rev. 27) dated 7/23/04 is applicable to any resultant contract. Offerors must include a completed copy of the provision at 52.212-3 Offeror Representations and Certifications-Commercial Items, with his/her offer or be registered in ORCA (for Reps and Certs) and Central Contractor Registration (CCR) (see http://www.bpn.gov or www.ccr.gov.) Original proposals shall be submitted to MAVIS NELSON, Contracting Officer (90C), VA Medical Center, Building 68, One Veterans Drive, Minneapolis, MN 55417 by 4:30 p.m. on June 1, 2005.
