- COMBINED SYNOPSIS/ SOLICITATION Cardiovascular Flowmeter RFQ 549-05-41 This is a combined synopsis/solicitation for commercial items prepared in accordance with subparts in the Federal Acquisition Regulation 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation and being issued as an anticipated sole source; quotations are being requested and a written solicitation will not be issued. This acquisition is issued as a Request for Quote. The combined Synopsis/solicitation number is _RFQ-549-05-41 This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-03. The Government intends to negotiate a commercial item (FAR Part 12) contract with Transonic Systems Inc. for supplies and one (1) Transonic System, Intraoperative Blood Flow Measurements HT314, for lease or buy to include all equipment maintenance/repair/and calibration. HT314 is single channel, ultrasonic transit time flowmeter with integral strip-chart prnter, FlowSound Audio Output: attached two meter extension cable and activated with a disposable Flow-QC- key into the Flowmeter. NOTE: The HT314 Flowmeter comes with 2 each 2MMs, 3MMs, and 4MMs coronary probes, and rolling stand. Probes are Non-Disposable and Compatible only with the HT314 Flowmeter. The above characteristics are intended to be descriptive, but not restrictive. VA North Texas Health Care System estimates 50 to 100 Coronary Artery Bypass Grafts (CABG) procedures per year. 2. Supplies consist of Flow QC Keys #KEY314-5, 5 per package. Full and Open competition with all responsible sources will be considered. Quotations shall be submitted ?on an all or none basis?. Partial quotations shall be rejected. All quotes must meet the listed requirements in this announcement. This procurement is unrestricted. Eventual contract award will be based on Best Value and vendors should be prepared to submit a quote that they feel will best support our requirements. The Government intends to award a firm - fixed price contract for a base year, with the Government's option to exercise two (2) one (1) Year option periods. Deliveries of the consumables and the HT314, Cardiovascular Flowmeter will be to the VANTHCS Medical Center, 4500 S. Lancaster Rd, Dallas, Texas 75216. NAICS #339112, Size Standard is 500 employees.
