Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation and a written solicitation will not be issued. The Request for Proposal number is N62645-05-R-0027. Provisions and clauses in effect through Federal Acquisition Circular 2005-03 are incorporated. NAICS 423450. The Naval Medical Logistics Command intends to negotiate (IAW FAR 6.302) with Haemonetics Corporation, 400 Wood Road, Braintree MA. as the only source that can provide Frozen Blood Cell Washing Systems and ancillary components. SALIENT CHARACTERISTICS: 1) FROZEN BLOOD CELL WASHER; The system shall be composed of a blood cell washer and the ancillary components necessary to operate the cell washer for the degylocerization of frozen packed red blood cells in a manner which allows use of the processed blood for a period not less than 12 days following processing and include installation. 2) REGULATORY REQUIREMENTS: The system, and all of the components, shall be compliant with the Food and Drug Administration requirements to market and deliver a medical product for use in the United States of America, even should delivery be requested outside of the United States. The claims made for the product shall comply with the regulations of the Food and Drug Administration with respect to products for marketing and delivery of a medical product for use in the United States of America, even should delivery be requested outside of the United States. 3) SYSTEM REQUIREMENTS: A. The system shall be cleared for marketing such that a unit of packed frozen red blood cells processed for freezing on the system can be thawed and processed for use with a useful life of not less than 12 days following the final processing, if appropriately refrigerated. B. The system shall be capable of glycerolizing fresh red blood cells to protect them from cellular damage when frozen to -80 degrees Celsius. C. The system shall be capable of reversing the above process to prepare the thawed, previously frozen, packed red blood cells for delivery into a living human?s bloodstream without adverse effects from the chemicals used. D. The system, and all of its components that require electrical power, shall operate from 115 volts, 60-hertz electrical sources. (When specifically requested on a delivery order, the system, and all of its components shall operate from 220 volts, 50-hertz electrical sources.). E. The system shall not require more than 16 cubic feet of space for operation. F. The system shall not require more than 60 minutes to process one unit of packed frozen red blood cells. G. The system shall be designed such that one operator can operate a minimum of 3 units simultaneously (starting sequentially). H. The system shall include an operators manual, a training program, a service manual and parts manual (providing sufficient information for an electronic technician to identify and order parts), any diagnostic software necessary for the troubleshooting, repair, and calibration of the unit, and any specialized test equipment necessary to troubleshoot, repair, or calibrate the unit. (Specialized test equipment is that which is specifically designed for this product, or only for the manufacturers products, and which is not commercially marketed as a standard commercial tool or test equipment item for the biomedical service industry.) One list of all required tools, lubricants or similar consumable materials, and test equipment necessary for servicing the product shall be either included in the service manual, or provided as a separate document, clearly label with the make an model of the equipment. The products shall be labeled in accordance with Mil-STD-130L. PROPOSAL REQUIREMENTS: (1) A document addressing compliance for each numbered paragraph of the SALIENT CHARACTERISTICS. This document shall be in sufficient detail to show compliance with all items specified under the numbered paragraphs. In the event of a "nonconcur?, an explanation must be provided to support. (2) Any brochures, technical or product literature to support the items offered. (3) For past performance, provide dates of contracts or purchase orders, both Government and commercial, for the systems sold commercially (minimum of 3 different locations) within the past 3 year period as follows: Name, address, POC and telephone number of the organization; contract or purchase order number and dollar value; date of contract or purchase order. (4) The government anticipates award of a Requirements type contract with a base year and four options years, for a total not to exceed five years. Companies must submit firm-fixed pricing for the base year and all option years, to include commercial price lists and any applicable discounts to the Government need to be stated, and if applicable, any GSA schedule numbers and pricing. The contract will specify a maximum estimated amount of 84 frozen blood cell washing systems, and ancillary components to include AS3 solution, no minimum, a maximum of 725 cases; processed cell kits, no minimum, a maximum of 1800 boxes; sterile docking station, no minimum, a maximum of 30 units. Contract award will be based on 1). Determination of technical acceptability of items offered, and 2). Determination of low price. Companies must be registered in CCR; provide DUNS number; Cage Code and TIN. Companies must also comply with DFARS 252.232-7003, ELECTRONIC SUBMISSION OF PAYMENT REQUESTS (JAN 2004). PROVISIONS at FAR 52.212-1, Instructions to Offerors---Commercial Items, apply to this acquisition with the exception of (d), (h), and (i) of the clause, which are RESERVED. Offeror shall include a completed copy of provision at FAR 52.212-3, Offeror Representations and Certifications-Commercial Items, with their offer. Offeror shall include a completed copy of provision at DFARS 252.212-7000, Offeror Representations and Certification-Commercial Items. Addenda to this provision includes DFARS 252.225-7000, Buy American Act-Balance of Payments Program Certificate; 252.225-7006, Buy American Act-Trade Agreements-Balance of Payments Program Certificate; and 252.225-7035, Buy American Act--North American Free Trade Agreement Implementation Act--Balance of Payments Program Certificate. FAR 52.212-4, Contract Terms and Conditions---Commercial Items, applies to this acquisition. FAR 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items, applies to this acquisition. In compliance with said clause, the following FAR clauses apply: 52.203-6, Restrictions on Subcontractor Sales to the Government, with Alternate I; 52.222-26, Equal Opportunity; 52.222-35, Affirmative Action for Special Disabled and Vietnam Era Veterans; 52.222-36, Affirmative Action for Handicapped Workers; 52.222-37, Employment Reports on Special Disabled Veterans and Veterans of the Vietnam Era.
