Description
This is a combined synopsis/solicitation for a commercial item prepared in accordance with the format in Federal Acquisition Regulation Subpart 13, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation. Proposals are being requested and a written solicitation will not be issued. Reference Number F2Y3CE5087A100 is issued as a request for quotation (RFQ). The incorporated provisions and clauses are those in effect through the Federal Acquisition Circular 2005-01. This solicitation is set-aside for small business. The associated Standard Industrial Classification Code (SIC) is 3317 and the North American Industry Classification System (NAICS) is 331210. The Small Business Size Standard is 6 Million. CLIN 0001 – Mobilize, and extract submersible pump unit and associated hardware and piping. Redevelop the well (bail and swab to cap at 828’ depth). Perform a video inspection of well casing and screen assembly for damage. After the evaluation is completed, the contractor shall submit a failure analysis report and a copy of video, with a written detailed cost proposal for repair of any damage. The failure analysis report and videotape shall be provided to the Contracting Officer within 45 days after evaluation. Negative reports are required. Quantity: 1 job Depth of Well = 828 feet Diameter of Well = 1 foot Depth to Pump = 350 feet Diameter/tpy or drop pipe (column) to pump = 8 inch WA State Department of Ecology well ID (tag) Number = Ecology ID (tag) AFC-535. Ecology Start Card # W121735; DOH ID is S12 Pump manufacturer model, Serial # = Goulds Lineshaft Submersible Model VIS, 75 hp, 3500 RPM Motor Model, Serial # = Franklin Submersible Motor NOTE: A copy of the well report will be provided upon request. CLIN 0002 – Cost to repair/replace Quantity: 1 job Price and Period of Performance to be determined. The Contracting Officer will respond in writing within 30 days to definitize the order to increase funding to allow for the completion of repairs. Period of Performance will be negotiated along with cost. CLIN 0003 – Perform pump test of well to establish maximum sustainable output. Re-install submersible pump, related hardware and adjust pump to maximum sustainable output. Period of Performance shall be completed within 15 days after government acceptance of negative report or completion of CLIN 0002 repairs. Quantity: 1 job A Not-To-Exceed purchase order will be awarded using Simplified Acquisition Procedures. Award will be made to the responsible small business offering the lowest price for CLIN 0001 & 0003, combined. Inspection Point: 62 CES/CEOIU, Bldg 301, 301 Pitsenbarger, McChord AFB, WA 98438. Partial payments will be authorized for CLIN 0001.
