- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The solicitation number is 41302-021-5 X-Ray and is issued as a Request for Quotes (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-03. This acquisition is a small business set-aside. The NAICS code for this requirement is 423450; the small business size standard is 100 employees. ///// GENERAL REQUIREMENTS: The Federal Correctional Institution located at 2300 County Road 29, Sandstone, MN 55072, is requesting quotes for the following items listed below. Item descriptions listed are based on specifications utilized from an Amrad High Frequency FMT System X-Ray Generator. This RFQ is issued incorporating FAR 52.211-6, Brand Name or Equal (Aug 1999). We require that any X-Ray Generator brands that are quoted to us be compatible with our current x-ray processor, an All-Pro 100+. All prices must be quoted FOB Destination. Delivery is required within 30 days of award. The Government intends to make a single award to the lowest-priced technically acceptable offer. ///// SCHEDULE OF ITEMS: (1) X-RAY GENERATOR: constant potential 37.5 kW power rating; high frequency =/> 100 kHz; radiographic kV range in 1kV steps: 40-125; mAs range: 0.5 - 400 mAs with 45 mAs selections; digital display of kVp, mAs, and mA; built-in self diagnostics and built-in tube protection; manual and automatic anatomical technique programming; operates from single phase 230-250V electrical service; UL classified. (2) STANDARD X-RAY TUBE: 1.0 - 2.0 mm focal spots, 140,000 heat units. (3) TUBESTAND: floor to wall, 10 ft. length rails; rotational base, 180 degrees with 90 degrees detents; transverse tubearm with +/- 5" travel, trunion ring mount; angulation dial, operator handgrips, and electric locks. (4) RADIOGRAPHIC TABLE: four-way float-top table with foot control; 5" transverse travel, 32" longitudinal travel; heavy-duty cassette tray; grid: 10:1 ratio, 103 lpi; 400 lb. rated patient load; 17" bucky with electric lock, 18.5" travel. (5) HIGH VOLTAGE CABLES: 25ft. with federal terminations. (6) COLLIMATOR: certified manual with light field, swivel mount. (7) WALLSTAND: wallstand, 17" x 17"; grid cabinet, 17" x 17"; grid: 10:1 ratio, 103 lpi; heavy-duty cassette tray, electric lock. ADDITIONAL REQUIREMENTS: Please note if the following are included in your price quote: (a) warranty. (b) installation, calibration, and in-servicing of equipment.
