Description

The contractor shall provide all management, supervision, labor, materials, supplies and equipment (except as otherwise provided), and shall plan, schedule, coordinate and ensure effective performance to provide complete Hospital Aseptic Management System services. The Contractor shall furnish all personnel, materials, supplies, machinery, and appliances necessary to provide janitorial service at the Tulsa Outpatient Clinic, 9322 E. 41st Street, Tulsa, Oklahoma 74145. The approximate area is 44,000 square feet. The Tulsa Outpatient Clinic is a medical treatment operation, which requires attention inherent to such a setting. All materials and supplies and equipment used in performance of this contract will be removed from the facility or stored properly at the end of the work day. The contractor shall ensure that equipment is kept is safe, good working condition, and cleaned and properly stored at the end of the work day. A walk through inspection of the medical facility is strongly suggested of any prospective bidders prior to bid opening. Post-award conferences will be held at a time and place designated by the Contracting Officer, to be attended by the Contractor, the Contractor?s supervisor, the Contracting Officer?s Technical Representative, the Contracting Officer, and other officials designated by the Contracting Officer. The Company must have experience in managing and performing a Hospital Aseptic Management System (HAMS) program. This experience must have been gained as a result of a company being regularly engaged in the business of providing aseptic services in all of the areas of a hospital or outpatient facility. The minimum acceptable experience is 24 months within the 36 months previous to the due date established in the solicitation. The NAICS code is 561720 and the business size is 14m. Requests for bid packages shall be requested on company letterhead and faxed to 918-680-3852, or mailed to Muskogee VAMC, 1011 Honor Heights Drive, Muskogee, OK 74401. Bid packages shall be due in this office no later than July 20, 2005 @ 4:30pm
