Description
The Federal Bureau of Prisons, Community Corrections Contracting is seeking a Request for Quote from small business concerns having the ability to provide outpatient substance abuse treatment and mental health services for federal offenders residing at the Community Corrections Center contract facilities (Dismas Charities North) located at 1010 West Peachtree Street, Atlanta, GA 30309 and (Dismas Charities West) located at 300 Wendell Court, Atlanta, GA 30336 and those that are under home confinement in the Atlanta, Georgia area. Due to traffic congestion and inmate residents? reliance on public transportation, the treatment facility shall be located within five (5) miles driving distance from the contract facility located at 1010 West Peachtree Street, Atlanta, GA 30309. Performance period and requirements under this contract will be: 1. Base period - 10/01/2005 - 09/30/2006: (a) Intake Assessment - 2 each, (b) Individual Counseling (30 minute session, 24 sessions per inmate X 38 inmates) = 912 sessions, ?) Group Counseling (30 minute session, 72 sessions per inmate X 38 inmates) = 2,736 sessions, (d) Family Counseling (30 minute session, 12 sessions per inmate X 6 inmates) = 72 sessions, (e) Mental Health Intake Assessments and Report = 6 each, (f) Medication Monitoring = 10 inmates, (g) Intensive Outpatient Counseling (30 minute session, 12 sessions per inmate X 6 inmates) = 72 sessions, (h) Mental Health Individual Counseling (30 minute session, 12 sessions per inmate X 6 inmates) = 72 sessions, (i) Mental Health Group Counseling (30 minute session, 12 sessions per inmate X 6 inmates) = 72 sessions; 2. Option Year 1 - 10/01/2006 - 09/30/2007:(a) Intake Assessment = 3 each, (b) Individual Counseling (30 minute session, (24 sessions per inmate X 40 inmates) = 960 sessions, ?) Group Counseling (30 minute session, 72 sessions per inmate X 40 inmates) = 2,880 sessions, (d) Family Counseling (30 minute session, 12 sessions per inmate X 6 inmates) = 72 sessions, (e) Mental Health Intake Assessments and Report = 6 each, (f) Medication Monitoring = 10 each, (g) Intensive Outpatient Counseling (30 minute session, 12 sessions per inmate X 6 inmates) = 72 sessions, (h) Mental Health Individual Counseling (30 minute session, 12 sessions per inmate X 6 inmates) = 72 sessions, (i) Mental Health Group Counseling (30 minute session, 12 sessions per inmate X 6 inmates) = 72 sessions; 3. Option Year 2 - 10/01/2007 - 09/30/2008: (a) Intake Assessment = 4 each, (b) Individual Counseling (30 minute session, 24 sessions per inmate X 42 inmates) = 1,008 sessions, ?) Group Counseling (30 minute session, 72 sessions per inmate X 42 inmates) = 3,024 sessions, (d) Family Counseling (30 minute session, 12 sessions per inmate X 6 inmates) = 72 sessions, (e) Mental Health Intake Assessments and Report = 6 each, (f) Medication Monitoring = 10 each, (g) Intensive Outpatient Counseling (30 minute session, 12 sessions per inmate X 6 inmates) = 72 sessions, (h) Mental Health Individual Counseling (30 minute session, 12 sessions per inmate X 6 inmates) = 72 sessions, (i) Mental Health Group Counseling (30 minute session, 12 sessions per inmate X 6 inmates) = 72 sessions; 4. Option Year 3 - 10/01/2008 - 09/30/2009: (a) Intake Assessment = 4 each, (b) Individual Counseling (30 minute session, 24 sessions per inmate X 44 inmates) = 1,056 sessions, ?) Group Counseling (30 minute session, 72 sessions per inmate X 44 inmates) = 3,168 sessions, (d) Family Counseling (30 minute session, (12 sessions per inmate X 6 inmates) = 72 sessions, (e) Mental Health Intake Assessments and Report = 6 each, (f) Medication Monitoring = 10 each, (g) Intensive Outpatient Counseling (30 minute session, 12 sessions per inmate X 6 inmates) = 72 sessions, (h) Mental Health Individual Counseling (30 minute session, 12 sessions per inmate X 6 inmates) = 72 sessions, (i) Mental Health Group Counseling (30 minute session, 12 sessions per inmate X 6 inmates) = 72 sessions; 5. Option Year 4 - 10/01/2009 - 09/30/2010: (a) Intake Assessment = 4 each, (b) Individual Counseling (30 minute session, 24 sessions per inmate X 46 inmates) = 1,104 sessions, ?) Group Counseling (30 minute session, 72 sessions per inmate X 46 inmates) = 3,312 sessions, (d) Family Counseling (30 minute session, 12 sessions per inmate X 6 inmates) = 72 sessions, (e) Mental Health Intake Assessments and Report = 6 each, (f) Medication Monitoring = 10 each, (g) Intensive Outpatient Counseling (30 minute session, (12 sessions per inmate X 6 inmates) = 72 sessions, (h) Mental Health Individual Counseling (30 minute session, 12 sessions per inmate X 6 inmates) = 72 sessions, (i) Mental Health Group Counseling (30 minute session, (12 sessions per inmate X 6 inmates) = 72 sessions. The services required above shall be in accordance with the Federal Bureau of Prisons Transitional Drug Abuse Treatment (TDAT) Statement of Work dated January 2004. The solicitation will be available on or before June 27, 2005 and will be distributed solely through the General Services Administration?s website http://www.fedbizopps.gov/. Hard copies of the solicitation document will not be available. The site provides downloading instructions. All future information about this acquisition including solicitation amendments will be distributed solely through this site. Interested parties are responsible for monitoring this site to ensure that they have the most up-to-date information about this acquisition. The estimated closing date is on or about July 26, 2005. This is a 100% small business set-aside acquisition under NAICS 621420 and small business size standard at $8.5 million. All responsible small business concerns (for profit) may submit a quote which will be considered by this agency.
